小针刀联合艾灸对慢性膝关节骨性关节炎患者疼痛及炎性因子的影响

The Influence of Small Acupotomy Combined with Moxibustion on Pain and Inflammatory Factors in Patients with Chronic Knee Osteoarthritis

目的:观察小针刀联合艾灸对阳虚寒凝型慢性膝关节骨性关节炎患者疼痛及炎性因子的影响。方法:选取2019年1月至2022年10月在河南大学第一附属医院收治的慢性膝关节骨性关节炎患者90例,按照随机数字表法分为观察组和对照组,每组45例。对照组给予针刺联合艾灸治疗,观察组给予小针刀联合艾灸治疗。观察两组患者治疗前后西安大略和麦克马斯特大学骨关节炎指数(western ontario and mcmaster universities osteoarthritis index, WOMAC)、疼痛视觉模拟评分(visual analogue score, VAS)及炎性因子[白细胞介素(interleukin, IL)-1β、IL-6和肿瘤坏死因子(tumor necrosis factor, TNF)-α]变化情况。结果:观察组治疗后WOMAC评分、VAS评分低于对照组,差异有统计学意义(P<0.05);观察组治疗后IL-1β、IL-6、TNF-α水平低于对照组,差异具有统计学意义(P<0.05)。结论:小针刀联合艾灸治疗慢性膝关节骨性关节炎,能够改善患者骨关节炎指数,缓解疼痛,降低炎性因子水平。

Objective:To observe the influence of small acupotomy combined with moxibustion on pain and inflammatory factors in patients with chronic knee osteoarthritis.Methods:A total of 90 patients with chronic knee osteoarthritis admitted to The First Affiliated Hospital of Henan University from January 2019 to October 2022 were randomly divided into the observation group and the control group,with 45 cases in each group.The control group was treated with acupuncture combined with moxibustion,while the observation group was treated with small acupotomy combined with moxibustion.The changes of Western Ontario and mcmaster universities osteoarthritis index(WOMAC),visual analog score(VAS)for pain,and inflammatory factors[including interleukin(IL-1)]-β,IL-6 and tumor necrosis factor(TNF)-α before and after treatment in two groups were observed.Results:After treatment,the WOMAC score and VAS score of the observation group were lower than those of the control group,and all the differences were statistically significant(P<0.05);The levels of IL-1β,IL-6,and TNF-α in the observation group were lower than those of the control group,and all the differences were statistically significant(P<0.05).Conclusion:The combination of small acupotomy and moxibustion in the treatment of chronic knee osteoarthritis can improve the patients'bone joint index,alleviate pain,and re duce the level of inflammatory factors.
