分析利培酮联合奥氮平治疗老年精神分裂症患者的效果及对机体糖脂代谢的影响

Effect of risperidone combined with olanzapine in the treatment of elderly patients with schizophrenia and their impact on glucose and lipid metabolism

目的分析老年精神分裂症患者采用利培酮联合奥氮平治疗的效果,并观察其对机体糖脂代谢的影响。方法60例老年精神分裂症患者,采用随机数字表法分为对照组及观察组,每组30例。对照组应用奥氮平治疗,观察组应用利培酮联合奥氮平治疗。对比两组患者治疗前后的阳性与阴性症状量表(PANSS)评分,临床疗效,治疗前后糖脂代谢指标[空腹血糖(FBG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)以及甘油三酯(TG)],药物不良反应发生情况。结果治疗前,两组PANSS评分对比,差异无统计学意义(P>0.05);治疗1、2个月后,两组PANSS评分均低于本组治疗前,且观察组的PANSS评分分别为(76.26±3.25)、(43.26±3.96)分,均低于对照组的(81.06±5.19)、(53.06±4.06)分,差异具有统计学意义(P<0.05)。观察组治疗总有效率96.67%高于对照组的80.00%,差异具有统计学意义(P<0.05)。两组治疗前后的FBG、HbA1c、TC、TG组间、组内对比,差异无统计学意义(P>0.05)。两组药物不良反应发生率对比,差异无统计学意义(P>0.05)。结论老年精神分裂症患者应用利培酮联合奥氮平治疗可取得满意的临床疗效,且对机体糖脂代谢造成的影响较小。

Objective To analyze the effect of risperidone combined with olanzapine in the treatment of elderly patients with schizophrenia,and observe their impact on glucose and lipid metabolism.Methods A total of 60 elderly patients with schizophrenia were divided into a control group and an observation group according to the random numerical table,with 30 cases in each group.The control group was treated with olanzapine,and the observation group was treated with risperidone and olanzapine.Both groups were compared in terms of positive and negative symptom scale(PANSS)score,clinical efficacy,glucose and lipid metabolism[fasting blood glucose(FBG),glycosylated hemoglobin(HbA1c),total cholesterol(TC),and triglycerides(TG)]before and after treatment,and the occurrence of adverse drug reactions.Results Before treatment,there was no statistically significant difference in PANSS score between the two groups(P>0.05).After 1 and 2 months of treatment,the PANSS scores in both groups were lower than those before treatment in this group;PANSS scores in the observation group were(76.26±3.25)and(43.26±3.96)points,which were lower than those of(81.06±5.19)and(53.06±4.06)points in the control group;the differences were statistically significant(P<0.05).The total effective rate of treatment in the observation group was 96.67%,which was higher than that of 80.00%in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in FBG,HbA1c,TC and TG before and after treatment between the two groups and within the groups(P>0.05).There was no statistically significant difference in the incidence of adverse drug reactions between the two groups(P>0.05).Conclusion Risperidone combined with olanzapine can achieve satisfactory clinical efficacy in elderly patients with schizophrenia,and have little effect on glucose and lipid metabolism.
