结直肠癌组织中HER2表达水平及其与患者临床病理特征和预后的相关性分析

Expression of HER-2 in colorectal cancer and its correlations with clinicopathological characteristics and prognosis

目的:探讨结直肠癌组织中人表皮生长因子受体2(HER2)表达水平及其与临床病理特征和预后的关系。方法:回顾性分析2016年1月至2021年1月该院收治的80例结直肠癌患者的临床资料,评估患者HER2表达情况,并统计临床病理特征,包括性别、病灶位置、年龄、病灶直径、临床分期、分化程度、淋巴结转移、病理类型、浸润深度、远处转移情况等资料。分析结直肠癌患者HER2表达情况及其与临床病理特征和预后的关系。结果:80例结直肠癌患者中,46例HER2阴性(26例为0,20例为1+),34例HER2阳性(22例为2+,12例为3+),阳性率为42.50%;不同临床分期、浸润深度、淋巴结转移情况、远处转移情况、分化程度患者的HER-2阳性率比较,差异有统计学意义(P<0.05);不同性别、年龄、病灶位置、病灶直径、肿瘤类型患者的HER-2阳性率比较,差异无统计学意义(P>0.05);Spearman相关性分析显示,HER-2表达水平与临床分期、浸润深度、淋巴结转移、远处转移、分化程度等病理特征均呈正相关(r=0.425、0.384、0.267、0.358、0.391,P<0.05);随访1~3年,阳性组生存率为55.88%(19/34),低于阴性组的84.78%(39/46),差异有统计学意义(P<0.05)。结论:HER2表达水平与临床分期、浸润深度、淋巴结转移、远处转移、分化程度等病理特征均呈正相关,HER-2阳性患者的生存率显著低于阴性患者。

Objective:To investigate expression of human epidermal growth factor receptor 2(HER2)in colorectal cancer and its relationships with clinicopathological characteristics and prognosis.Methods:Clinical data of 80 patients with colorectal cancer admitted to the hospital from January 2016 to January 2021 were retrospectively analyzed.The expression of HER2 was evaluated,and clinicopathologic features including sex,location,age,diameter,clinical stage,degree of differentiation,lymph node metastasis,pathological type,depth of invasion,and distant metastasis were counted.The expression of HER2 in the patients with colorectal cancer and its relationships with clinicopathological characteristics and prognosis were analyzed.Results:Of the 80 patients with colorectal cancer,46 cases were negative(26 cases 0 and 20 cases 1+)and 34 cases were HER2-positive(22 cases 2+and 12 cases 3+).HER2 expression was correlated with clinical stage,lymph node metastasis,degree of differentiation,depth of invasion,distant metastasis and other pathological features,with statistical significance(P<0.05).HER2 expression was not correlated with gender,age,lesion location,pathological type,lesion diameter and other pathological characteristics,and there was no statistical significance(P>0.05).Spearman correlation analysis showed that the expression of HER2 was significantly positively correlated with the pathological characteristics such as distant metastasis,clinical stage,degree of differentiation,depth of invasion,and lymph node metastasis(r>0,P<0.05).All patients completed the follow-up.The survival rate in the positive group was 55.88%(19/34),which was lower than that the negative group of 84.78%(39/46),and the difference was statistically significant(P<0.05).Conclusions:The expression level of HER2 is positively correlated with clinical stage,depth of invasion,lymph node metastasis,distant metastasis,and degree of differentiation.Further,the survival rate of the HER-2 positive patients is significantly lower than that of the negative patients.
