肌功能训练联合功能矫治器在安氏Ⅱ类错[牙合]畸形患儿正畸治疗中的应用效果 被引量:3

Effects of muscle function training combined with functional appliance in orthodontic treatment of children with Angle’s class Ⅱ malocclusion

摘要 目的:探讨肌功能训练联合功能矫治器在安氏Ⅱ类错[牙合]畸形患儿正畸治疗中的应用效果。方法:回顾性分析2019年2月至2021年8月该院收治的60例安氏Ⅱ类错[牙合]畸形患儿的临床资料,根据治疗方法不同将患儿分为观察组与对照组各30例。对照组采用FrankelⅡ型矫治器治疗,观察组在对照组基础上联合肌功能训练。矫治1年后,比较两组患儿的矫治效果,矫治前后X线头影测量指标[上齿槽座夹角(SNA角)、下齿槽座夹角(SNB角)、上中切牙长轴与前颅底平面夹角(U1-SN角)、上中切牙倾斜度(U1-NA角)、前牙覆盖(OJ)、覆[牙合](OB)及下颌平角面(MP-FH角)],以及颞下颌关节参数。结果:观察组矫治总有效率为93.33%(28/30),明显高于对照组的66.67%(20/30),差异有统计学意义(P<0.05);矫治1年后,观察组SNA角、SNB角明显大于对照组,U1-NA角、U1-SN角、OJ、OB、MP-FH角明显小于对照组,颞下颌关节上、关节后间隙均大于对照组,差异有统计学意义(P<0.05);两组关节前间隙比较,差异无统计学意义(P>0.05)。结论:肌功能训练联合FrankelⅡ型功能矫治器用于安氏Ⅱ类错[牙合]畸形患儿中,能够更好地改善患儿上下颌骨关系,改善侧貌面型,明显提高患儿矫治效果。 Objective:To explore effects of muscle function training combined with functional appliance in orthodontic treatment of children with Angle’s class Ⅱ malocclusion.Methods:The clinical data of 60 children with Angle’s class Ⅱ malformation admitted to this hospital from February 2019 to August 2021 were retrospectively analyzed.According to different treatment methods,these children were divided into observation group and control group,30 cases in each group.The control group was treated with Frankel Ⅱ appliance,while the observation group was combined with muscle function training on the basis of that of the control group.After 1 year of orthodontic treatment,the correction effects,the X-ray cephalometric index levels before and after the treatment[sella nasion A point angle(SNA angle),sella nasion B point angle(SNB angle),angle between maxillary central incisor long axis and anterior cranial base plane(U1-SN angle),maxillary central incisor inclination(U1-NA angle),anterior overjet(OJ),overbite(OB)and mandibular plane angle(MP-FH angle)],and the temporomandibular joint parameter levels were compared between the two groups.Results:The total effective rate of correction in the observation group was 93.33%(28/30),which was significantly higher than 66.67%(20/30)in the control group,and the difference was statistically significant(P<0.05).After 1 year of treatment,the SNA angle and the SNB angle of the observation group were significantly larger than those of the control group;the U1-NA distance,the U1-SN angle,the OJ,the OB and the MP-FH angle were significantly smaller than the control group;the superior and posterior joint space levels of temporomandibular joint were larger than those of the control group;and the differences were statistically significant(P<0.05).However,there was no significant difference in the anterior joint space between the two groups(P>0.05).Conclusions:Muscle function training combined with Frankel type Ⅱ functional appliance for the children with Angle’s class Ⅱ malocclusion can better improve the relationship between maxilla and mandible,improve the lateral profile,and significantly improve the treatment effects.
