布托啡诺经鼻给药对全麻诱导期舒芬太尼诱发呛咳反应的预防作用

Preventive effect of butorphanol nasal administration on sufentanil induced cough re-sponse during induction of general anesthesia

目的 探究布托啡诺经鼻给药对全麻诱导期舒芬太尼诱发呛咳反应的预防作用。方法 将择期手术患者120例均分为对照组(全麻诱导前2 min静脉注射生理盐水1 mL)、静脉注射组(全麻诱导前2 min静脉注射布托啡诺1 mg),经鼻低剂量组(全麻诱导前15 min经鼻给与布托啡诺1 mg),经鼻高剂量组(全麻诱导前15 min经鼻给与布托啡诺2 mg)。观察各组患者呛咳反应发生率及严重程度;监测各组患者生命体征、临床指标及术后不良事件的发生情况。结果 静脉注射组、经鼻低剂量组、经鼻高剂量组呛咳反应发生率低于对照组(P<0.05);患者全麻苏醒及术后不良事件差异无显著性(P>0.05)。结论 高剂量布托啡诺经鼻给药对舒芬太尼诱发的呛咳反应有预防作用,不影响患者全麻苏醒,且术后无不良事件。

Aim To investigate the preventive effect of butorphanol nasal administration on cough reaction induced by sufentanil during induction of general anesthesia.Methods 120 patients undergoing elective surgery were randomly divided into the control group(1 mL saline was injected intravenously 2 minutes before induction of general anesthesia),the intravenous injection group(butorphanol 1 mg was injected intravenously 2 minutes before induction of general anesthesia),the low-dose nasal group(butorphanol 1 mg was administered intravenously 15 minutes before induction of general anesthesia),the high-dose nasal group(butorphanol 2 mg was administered intravenously 15 minutes before induction of general anesthesia).The incidence and severity of cough reactions in patients in each group were observed.Incidence of vital signs,clinical indicators,and postoperative adverse events in each group were monitored.Results The incidence of choking reaction induced by sufentanil in the Intravenous group,Low-dose nasal group and High-dose nasal group was lower than that in Control group,and the difference was statistically significant(P<0.05).There was no significant difference in recovery from general anesthesia and postoperative adverse reactions(P>0.05).Conclusion Nasal administration of butorphanol had a preventive effect on sufentanil-induced cough,did not affect the patient􀆳s recovery from general anesthesia,and there were no postoperative adverse events.
