射血分数保留的心力衰竭患者死亡风险影响因素的荟萃分析

Possible items affecting mortality in patients with heart failure with preserved ejection fraction:a meta-analysis

目的分析影响射血分数保留的心力衰竭(HFpEF)患者临床预后的各种危险因素,为改善临床预后提供理论依据。方法通过全面检索Pubmed、Web of science、Embase和Cochrane library等数据库,筛选关于影响HFpEF保留的心力衰竭患者死亡率相关影响因素的文献进行荟萃分析。数据分析在Stata 16.0软件进行操作。结果本研究一共纳入11项临床研究。统计结果提示年龄(OR=1.05,95%CI:1.03~1.08)、脑钠肽(OR=1.35,95%CI:1.28~1.42)、心房颤动(OR=2.17,95%CI:1.65~2.86)、贫血(OR=2.43,95%CI:1.25~4.74)为HFpEF患者的死亡因素,体重指数(OR=0.96,95%CI:0.93~1.00)为HFpEF患者的保护性因素。结论年龄、脑钠肽、心房颤动、贫血是HFpEF患者死亡的危险因素,而体重指数却为HFpEF患者死亡的保护性因素,可为临床及早进行针对性干预及改善预后提供一定的理论依据。

Objective To analyze various risk factors affecting the clinical prognosis of heart failure with preserved ejection fraction(HFpEF)and to provide a theoretical basis for improving clinical prognosis.Methods A comprehensive search of PubMed,Web of Science,Embase and Cochrane library databases was performed to screen the literature on the influencing factors related to mortality in HFpEF for meta-analysis.Data analysis was performed in Stata 16.0 software.Results A total of 11 clinical studies were included in this study.Statistical results suggested age(OR=1.05,95%CI:1.03-1.08),brain natriuretic peptide(OR=1.35,95%CI:1.28-1.42),atrial fibrillation(OR=2.17,95%CI:1.65-2.86)and anemia(OR=2.43,95%CI:1.25-4.74)were mortality factors in patients with HFpEF,however,the body mass index results suggested that(OR=0.96,95%CI:0.93-1.00)was a protective factor.Conclusion Age,brain natriuretic peptide,atrial fibrillation,and anemia are risk factors for death in HFpEF,while body mass index is a protective factor for death.Our findings will provide some theoretical basis for early clinical targeted intervention and improved prognosis.
