卡前列素氨丁三醇联合麦角新碱在剖宫产术后出血中的应用效果

Application effect of Carboprost Tromethamine combined with Ergometrine in hemorrhage after cesarean section

目的探究卡前列素氨丁三醇联合麦角新碱在剖宫产术后出血中的应用效果。方法选取2018年6月至2021年9月辽阳市第三人民医院60例剖宫产后出血的患者作为研究对象,按照随机数字表法分为对照组与观察组,各30例。两组患者均接受改良B-Lynch缝合术,对照组采用单纯卡前列素氨丁三醇干预,观察组采用卡前列素氨丁三醇联合麦角新碱干预。比较两组的产后出血量、手术时间、住院时间、临床疗效、宫底高度、宫缩次数、血性恶露时间及并发症发生情况。结果观察组产后2 h出血量、产后1 d总出血量少于对照组,住院时间短于对照组,差异有统计学意义(P<0.05),两组的手术时间比较,差异无统计学意义(P>0.05);观察组的总有效率高于对照组,差异有统计学意义(P<0.05);两组宫底高度、宫缩次数组间、时间及交互作用差异均有统计学意义(P<0.05)。观察组患者术后1、2 h的宫底高度低于对照组,宫缩次数多于对照组同期,差异有统计学意义(P<0.05);两组并发症总发生率及血性恶露时间比较,差异无统计学意义(P>0.05)。结论改良B-Lynch缝合技术联合卡前列素氨丁三醇+麦角新碱应用于剖宫产术疗效确切,降低患者出血量。

Objective To explore the application effect of Carboprost Tromethamine combined with Ergometrine in postoperative bleeding after cesarean section.Methods A total of 60 patients with hemorrhage after cesarean section who were admitted to Liaoyang Third People's Hospital from June 2018 to September 2021 were selected as research objects,and divided into control group and observation group by random number table method,with 30 cases in each group.Patients in both groups were treated with modified B-Lynch suture.The control group was treated with Carboprost Tromethamine alone,while the observation group was treated with Carboprost Tromethamine combined with Ergometrine.The postpartum blood loss,operation time,hospital stay,clinical efficacy,fundus height,number of contractions,lochia rubra time and the incidence of complications were compared between the two groups.Results The 2 h postpartum blood loss and 1 d postpartum total blood loss in the observation group were less than those in the control group,and the length of hospital stay was shorter than that in the control group,the differences were statistically significant(P<0.05),but there was no significant difference in the operation time between the two groups(P>0.05).The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).There were statistically significant differences in the height of the base of the uterus,the number of contractions,the time and interaction between the two groups(P<0.05).The fundal height of observation group was lower than that of control group 1 and 2 h after operation,and the number of contractions was more than that of control group,the differences were statistically significant(P<0.05).There was no significant difference in the total incidence of complications and lochia rubra time between the two groups(P>0.05).Conclusion Applying B-Lynch suture combined with Hemabate and ergometrine in cesarean section can reduce the patients'blood loss.
