肺癌患者术后发生VTE的相关影响因素

Related Influencing Factors of VTE in Postoperative Patients with Lung Cancer

目的 分析肺癌患者术后静脉血栓栓塞症(VTE)发生的相关影响因素。方法 选取接受行手术治疗的84例肺癌患者,统计其术后VTE发生情况;收集患者的年龄、性别等资料,以多因素Logistic回归分析模型分析VTE发生的影响因素。结果 84例肺癌患者,术后共有18例发生VTE,发生率为21.43%(18/84)。单因素分析显示:性别、体重指数(BMI)、病理类型、冠心病、住院时间与肺癌患者术后发生VTE无关,差异无统计学意义(P>0.05);而年龄、临床分期、吸烟史、D-二聚体(D-D)、术前化疗、高血压、饮酒史与肺癌患者术后发生VTE有关,差异有统计学意义(P<0.05);多因素Logistic回归分析显示:年龄≥60岁、Ⅲ~Ⅳ期、有吸烟史、D-D异常、术前行化疗、有高血压、有饮酒史为肺癌患者术后发生VTE的独立危险因素(P<0.05)。结论 肺癌术后患者VTE发生率较高,其发生与年龄≥60岁、Ⅲ~Ⅳ期、有吸烟史、D-D异常、术前行化疗、高血压及饮酒史有关,临床需据此需采取有效的防治措施。

Objective To analyze the factors related to the occurrence of venous thromboembolism(VTE)in postoperative patients with lung cancer.Methods 84 patients with lung cancer who received surgical treatment were selected and the incidence of VTE after surgery was counted;The age,sex and other data of patients were collected,and the factors affecting the occurrence of VTE were analyzed by multivariate logistic regression analysis model.Results Of the 84 patients with lung cancer,18 patients developed VTE after operation,with an incidence of 21.43%(18/84).Univariate analysis showed that sex,body mass index(BMI),pathological type,coronary heart disease,and length of stay were not related to postoperative VTE in patients with lung cancer,and the difference was not statistically significant(P>0.05);However,age,clinical stage,smoking history,D-dimer(D-D),preoperative chemotherapy,hypertension and drinking history were related to postoperative VTE in patients with lung cancer(P<0.05);Multivariate logistic regression analysis showed that age≥60 years old,stageⅢ~Ⅳ,smoking history,D-D abnormality,preoperative chemotherapy,hypertension and drinking history were independent risk factors for VTE in patients with lung cancer after surgery(P<0.05).Conclusion The incidence of VTE in patients with lung cancer after surgery is high.The incidence of VTE is related to age≥60 years old,stageⅢ~Ⅳ,smoking history,D-D abnormality,preoperative chemotherapy,hypertension and drinking history.Effective prevention and treatment measures should be taken according to this.
