急性白血病患儿化疗期间院内感染的临床特点及Logistic回归分析

Clinical Characteristics and Logistic Regression Analysis of Nosocomial Infection During Chemotherapy in Children with Acute Leukemia

目的 分析急性白血病患儿化疗期间院内感染的临床特点及危险因素。方法 选取急性白血病患儿132例作为研究对象,采用我院自制调查问卷收集所有患儿性别、年龄、白血病类型、住院时间、白细胞计数、中性粒细胞计数、免疫调节剂应用、侵入性操作等信息,分析急性白血病患儿化疗期间院内感染的影响因素。结果 急性白血病患儿化疗期间院内感染率为30.30%,感染部位以呼吸道、泌尿系统为主(P<0.05);住院时间、白细胞计数、中性粒细胞计数、有无侵入性操作与免疫调节剂应用是影响急性白血病患儿化疗期间院内感染的单因素(P<0.05);logistic回归分析显示,住院时间>30 d、白细胞计数低、中性粒细胞计数低、侵入性操作是急性白血病化疗期间院内感染的危险因素,免疫调节剂应用是急性白血病患儿化疗期间院内感染的保护因素(P<0.05)。结论 急性白血病患儿化疗期间院内感染的影响因素较多,在临床治疗时可依据上述危险因素予以针对性防治感染,进而降低急性白血病患儿化疗期间院内感染发生率。

Objective To analyze the clinical characteristics and risk factors of nosocomial infection during chemotherapy in children with acute leukemia.Methods 132 cases of children with acute leukemia were selected,using our homemade questionnaire collected gender,age,leukemia type,length of hospitalization,leukocyte count,neutrophil count,immunomodulator application,invasive operation information,analyzed the factors of nosocomial infection in children with acute leukemia during chemotherapy.Results The nosocomial infection rate in children with acute leukemia was 30.30%,The infection site was mainly respiratory tract and urinary system(P<0.05);Length of hospitalization,white blood cell count,neutrophil count,no invasive operation and immunomodulator application were the single factors affecting nosocomial infection during chemotherapy in children with acute leukemia(P<0.05);The logistic regression analysis revealed that,Time of stay>30d,low white blood cell count,low neutrophil count,when invasive procedures are risk factors for nosocomial infection during chemotherapy for acute leukemia,Immunomodulator application was a protective factor for nosocomial infection during chemotherapy in children with acute leukemia(P<0.05).Conclusion There are many factors affecting nosocomial infection during chemotherapy,and the infection can be prevented according to the above risk factors in clinical treatment,thus reducing the incidence of nosocomial infection during chemotherapy in children with acute leukemia.
