计算机断层扫描联合血清三叶因子1、微RNA-130a-5p检测对肺癌的诊断价值分析

摘要 目的 探究计算机断层扫描(CT)联合血清三叶因子1(TFF1)、微RNA-130a-5p(miR-130a-5p)检测对肺癌的诊断价值。方法 选取2015年2月至2016年9月石家庄市人民医院收治的94例肺癌病人为肺癌组,85例同期确诊的肺良性病变病人为良性病变组。对所有受试者进行CT检查,并分析CT对肺癌病人的诊断效能;采用酶联免疫吸附法(ELISA)检测血清TFF1水平;采用实时荧光定量PCR(qRT-PCR)检测血清miR-130a-5p相对表达量;ROC曲线分析血清TFF1、miR-130a-5p对肺癌的诊断价值;分析CT、TFF1、miR-130a-5p单独检测及联合检测对肺癌的诊断价值。结果 CT诊断肺癌误诊16例,19例漏诊。CT诊断肺癌的灵敏度为79.79%,特异度为81.18%,准确度为80.45%,误诊率、漏诊率分别为18.82%与20.21%。与良性病变组相比,肺癌组病人血清TFF1显著上升,miR-130a-5p水平显著下降(P<0.05)。血清TFF1、miR-130a-5p对肺癌诊断价值的曲线下面积(AUC)分别为0.85、0.83,截断值分别为1.17μg/L、0.72。与联合检测相比,CT检测的灵敏度、准确度显著降低,漏诊率显著升高;血清miR-130a-5p检测的特异度、准确度均显著降低,误诊率显著升高(P<0.05)。结论 肺癌病人血清TFF1、miR-130a-5p呈异常表达,CT联合血清TFF1、miR-130a-5p可提高肺癌诊断的灵敏度和特异度,可为肺癌的诊治提供参考依据。 Objective To explore the value of computed tomography(CT)combined with serum trefoil factor 1(TFF1)and miR-130a-5p in the diagnosis of lung cancer.Methods A total of 94 lung cancer patients admitted to Shijiazhuang People's Hospital from February 2015 to September 2016 were selected as the lung cancer group,another 85 patients with benign lung lesions were selected as the benign lesion group.CT examinations were performed on all subjects and the diagnostic efficacy of CT for lung cancer patients was analyzed.Enzyme-linked immunosorbent assay(ELISA)was used to check the level of serum TFF1 in patients,and real-time fluorescence quantification PCR(qRT-PCR)was used to detect the relative expression level of serum miR-130a-5p in patients.ROC curve was used to analyze the diagnostic value of serum TFF1 and miR-130a-5p for lung cancer.The diagnostic values of CT,serum TFF1,miR-130a-5p alone and combined detection for lung cancer were analyzed.Results Sixteen cases of lung cancer diagnosed by CT were misdiagnosed and 19 cases were missed.The sensitivity of CT in diagnosing lung cancer was 79.79%,the specificity was 81.18%,and the accuracy was 80.45%.The misdiagnosis rate and missed diagnosis rate were 18.82%and 20.21%,respectively.Compared with the benign lesion group,the serum TFF1 of patients in the lung cancer group increased significantly,while the level of miR-130a-5p de‐creased significantly(P<0.05).The areas under the curve(AUCs)of serum TFF1 and miR-130a-5p for diagnosing lung cancer were 0.85 and 0.83,respectively,and the cut-off values were 1.17μg/L and 0.72,respectively.Compared with the combined detection of the three,the sensitivity and accuracy of CT detection were significantly reduced,and the missed diagnosis rate was significantly increased;the specificity and accuracy of serum miR-130a-5p detection were significantly reduced,and the misdiagnosis rate was significantly in‐creased(P<0.05).Conclusions Serum TFF1 and miR-130a-5p are abnormally expressed in patients with lung cancer.CT combined with serum TFF1 and miR-130a-5p detection can improve the sensitivity and specificity of lung cancer diagnosis,which can provide a reference for the diagnosis and treatment of lung cancer in clinical practice.
