基于“中医体质”理论辨析针刺治疗围绝经期干眼的思路

Analysis on the acupuncture treatment of perimenopausal dry eye based on the theory of"Traditional Chinese Medicine constitution"

《黄帝内经》中蕴含的中医体质理论对眼科临床诊治具有指导意义。干眼为常见的眼科疾病之一,中医将其归为“白涩症”“神水将枯”“干涩昏花”等范畴。本文通过分析围绝经期妇女的体质特点,发现围绝经期干眼在病因方面以阴阳失调为主,而肾气日衰,天癸将竭,冲任亏虚,肾之阴阳失衡是主要病机。治疗方面,以平衡阴阳为法,根据虚损、阻滞之不同施以补、泻手法。选穴方面,宜在眼周选穴基础上结合经络辨证、脏腑辨证。本文通过探讨《黄帝内经》“中医体质”理论在围绝经期干眼治疗中的应用,以期完善本病的相关针刺治疗方案,为临床治疗提供新思路。

Traditional Chinese Medicine(TCM)constitution theory contained in Huangdineijing has guiding significance for clinical diagnosis and treatment of ophthalmology.Dry eye is one of the common ophthalmic diseases,which is classified as"white astringent disease""holy water will wither""dry,astringent and faint"in TCM.By analyzing the physical characteristics of perimenopausal women,this paper discovers that the main cause of dry eye in perimenopausal women is the imbalance of Yin and Yang,and the main pathogenesis is the decline of kidney Qi,the exhaustion of menstruation,the deficiency of Chong and Ren,and the imbalance of kidney Yin and Yang.In terms of treatment,the method of balancing Yin and Yang is adopted,and the reinforcing methods as well as the reducing methods are applied according to the difference of deficiency and obstruction.In terms of point selection,it is advisable to combine meridian syndrome differentiation and viscera syndrome differentiation on the basis of point selection around the eyes.This paper discusses the application of TCM constitution theory in the treatment of perimenopausal dry eye in Huangdineijing,in order to improve the relevant acupuncture treatment scheme for this disease and provide new ideas for clinical treatment.
