探讨穴位按摩联合缩宫素鼻喷雾剂在促进产妇泌乳中的效果

The effect of acupoint massage combined with oxytocin nasal spray on promoting lactation of parturient women

目的:探讨穴位按摩联合缩宫素鼻喷雾剂在促进产妇产后早开奶的效果。方法:选取2019年10月—2020年5月分娩的产妇200例,按随机数字表法分为观察组和对照组,每组100例。观察组采用穴位按摩联合缩宫素鼻喷雾剂;对照组采用穴位按摩。记录两组开始泌乳时间,测量泌乳量,采用疼痛视觉模拟评分法(VAS)评价产妇乳房胀痛程度,观察产妇母乳喂养依从性和母乳喂养情况。结果:观察组开始泌乳时间<24 h及≤72 h占比均显著高于对照组(P<0.05);观察组24 h, 48 h, 72 h泌乳量均显著高于对照组(P<0.05);观察组乳房胀痛为无痛或轻度疼痛占68%、中度疼痛占30%、重度疼痛占2%,显著优于对照组(42%,48%,10%)(P<0.05);两组产妇乳头直接喂养占56%、单侧哺乳时间每次>30 min占45%、哺乳频率每天≥8次占36%,显著高于对照组(30%,20%,13%)(P<0.05);观察组完全母乳喂养占59%,尝试母乳喂养失败占5%,显著优于对照组(34%,26%)(P<0.05)。结论:采用穴位按摩联合缩宫素鼻喷雾剂对产妇产后早开奶效果明显,促进产妇泌乳,提高产妇母乳喂养依从性,改善乳房胀痛及母乳喂养状态。

Objective:To explore the effect of acupoint massage combined with oxytocin Nasal Spray on promoting early postpartum lactation of parturient.Methods:All 200 cases of parturients in hospital from October 2019 to May 2020 were randomly divided into observation group(100 cases),acupoint massage plus Oxytocin Nasal Spray,and control group(100 cases).The observation group was treated with point massage combined with oxytocin nasal spray,while the control group was treated with point massage.The onset time of lactation was recorded,the amount of lactation was measured,and the pain degree of lactation was evaluated by visual analogue scale(VAS).Results:The proportion of lactation time<24 h and≤72 h in the observation group was significantly higher than that in the control group(P<0.05).The lactation volume of the observation group at 24 h,48 h and 72 h was significantly higher than that of the control group.The proportion of increased milk production at 24 h,48h and 72 h in the observation group was higher than that in the control group(P<0.05).In the breast swelling pain reaction of the observation group,no pain or mild pain accounted for 68%,moderate pain accounted for 30%,and severe pain accounted for 2%,which was significantly better than that of the control group(42%,48%,10%)(P<0.05).In the two groups,56%of the parturients were fed directly with nipples,45%of the parturients were fed with unilateral lactation for more than 30 minutes per time,and 36%of the parturients were fed with lactation frequency≥8 times per day,which was significantly higher than that in the control group(30%,20%,13%)(P<0.05).The observation group had 59%complete breastfeeding and 5%failed to try breastfeeding,which was significantly better than the control group(34%and 26%)(P<0.05).Conclusion:Acupoint massage combined with oxytocin nasal spray has good effect on early postpartum milk,promotes lactation,improves breastfeeding compliance,improves breast distention and breastfeeding.
