Study on the Relationship Between Inflammation Markers and Etiology in Hospitalized Children with Lower Respiratory Tract Infection at Different Ages

摘要 目的:探讨不同年龄段下呼吸道感染住院患儿炎症标志物与病原学的关系。方法:选择2021年4月至2022年8月我院收治的302例下呼吸道感染住院患儿为研究对象。参考患儿年龄,将年龄<3岁的130例患儿其分为婴幼儿组;将年龄≥3岁且<6岁的143例患儿纳入学龄前期组;将年龄≥6岁的29例患儿纳入学龄期组。分别比较三组患儿炎性标志物水平与病原体分布情况,并分析不同年龄段下呼吸道感染住院患儿炎症标志物与病原学关系。结果:婴幼儿组患儿病原体分布以细菌感染(60.00%)占比最高;学龄前期组患儿病原体分布以肺炎支原体感染(60.14%)占比最高;学龄期组患儿病原体分布以混合感染(82.76%)占比最高。学龄期组白细胞、淋巴细胞、血小板计数及CRP水平均高于婴幼儿组与学龄前期组,粒细胞水平低于婴幼儿组与学龄前期组(P<0.05)。混合感染患儿白细胞、淋巴细胞、血小板计数及CRP水平均细菌感染与肺炎支原体感染患儿,粒细胞水平低于细菌感染与肺炎支原体感染患儿(P<0.05)。结论:下呼吸道感染的婴幼儿与学龄期患儿患病后炎症反应较为严重,病原学主要分布为细菌感染与混合感染;临床通过监测患儿炎症指标变化可为其病原学类别的预测与后续治疗方案的制定提供数据参考。 Objective:To explore the relationship between inflammation markers and etiology in hospitalized children with lower respiratory tract infection in different age groups.Method:A total of 302 hospitalized children with lower respiratory tract infection in our hospital from April 2021 to August 2022 were selected as the study objects.According to the age of the children,130 children<years old were divided into infant group.A total of 143 children aged≥3 years and<6 years were included in the pre-school age group.A total of 29 school-age children aged≥6 years were enrolled in the study group.The levels of inflammatory markers and the distribution of pathogens in the three groups were compared,and the relationship between inflammatory markers and etiology in hospitalized children with lower respiratory tract infection in different ages was analyzed.Results:Bacterial infection(60.00%)accounted for the highest proportion of pathogen distribution in infant group.Mycoplasma pneumoniae infection(60.14%)accounted for the highest pathogen distribution in the early school-age group.Mixed infection(82.76%)accounted for the highest proportion of pathogens in school-age children.The levels of leukocyte,lymphocyte,platelet count and CRP in school-age group were higher than those in infant group and pre-school age group,and the level of granulocyte was lower than that in infant group and pre-school age group(P<0.05).The levels of leukocyte,lymphocyte,platelet count and CRP in the children with mixed infection were all lower than those in the children with bacterial infection and mycoplasma pneumoniae infection(P<0.05).Conclusions:The inflammatory reaction of school-age children with lower respiratory tract infection is more serious.The etiology is mainly bacterial infection and mixed infection.Clinical monitoring of inflammatory indicators in children can provide data reference for prediction of etiology category and formulation of follow-up treatment.
