耐药结核病患者营养状态评估及营养支持前后免疫指标对比

Nutritional Status Assessment and Comparison of Immune Indexes Before and After Nutritional Support in Patients with Drug-Resistant Tuberculosis

目的:探讨耐药结核病患者营养状态,分析营养支持治疗对此类患者营养指标与免疫指标水平的影响。方法:选择2018年8月至2022年8月我院收治的112例耐药结核病患者纳入观察组;另选择同期于我院体检的60名健康人员纳入对照组。使用随机数字表法将所有患者分为观察A组与观察B组。观察组A组应用常规抗结核、对症治疗;观察组B组在此基础上应用营养支持治疗。比较两组营养指标水平;比较两亚组营养指标与免疫指标水平。结果:观察组ALB、TP及Hb水平均低于对照组(P<0.05)。观察B组营养指标(ALB、TP、Hb)水平均高于观察A组,且与治疗前比较有明显差异(P<0.05)。治疗后,观察B组免疫指标(CD3^(+)、CD4^(+)、CD8^(+)与CD4^(+)/CD8^(+))水平均优于观察A组,与治疗前有明显差异(P<0.05)。结论:耐药结核病患者患病期间机体营养状态失衡,对此类患者实施营养支持治疗可显著提高其营养水平,改善患者免疫功能,为疾病持续康复创造有利条件,值得推广应用。

Objective:To investigate the nutritional status of patients with drug-resistant tuberculosis,and to analyze the effects of nutritional support therapy on the levels of nutritional indexes and immune indexes in these patients.Methods:112 patients with drug-resistant tuberculosis admitted to our hospital from August 2018 to August 2022 were included in the observation group.Another 60 healthy persons who underwent physical examination in our hospital during the same period were included in the control group.All patients were divided into observation group A and observation group B using random number table method.Observation group A received conventional anti-tuberculosis and symptomatic treatment;Observation group B received nutritional support therapy on this basis.The levels of nutritional indexes were compared between the two groups.The levels of nutritional and immune indexes were compared between the two subgroups.Results:The levels of ALB,TP and Hb in observation group were lower than those in control group(P<0.05).The levels of nutritional indexes(ALB,TP,Hb)in observation group B were higher than those in observation group A,and there were significant differences compared with before treatment(P<0.05).After treatment,the levels of immune indexes(CD3^(+),CD4^(+),CD8^(+) and CD4^(+)/CD8^(+))in group B were better than those in group A,and there were significant differences compared with before treatment(P<0.05).Conclusion:The nutritional status of drug-resistant TB patients is unbalanced during their illness.The implementation of nutritional support therapy for such patients can significantly improve their nutritional level,improve their immune function,and create strong conditions for the continuous recovery of the disease,which is worthy of popularization and application.
