宫腔镜冷刀切除术后给予左炔诺孕酮宫内缓释系统对子宫内膜息肉疗效的影响

Effect of Levonorgestrel Intrauterine Release System on Endometrial Polyps after Hysteroscopic Cold Knife Resection

目的:探讨宫腔镜冷刀切除术后予以左炔诺孕酮宫内缓释系统(LNG-IUS)治疗子宫内膜息肉的临床效果。方法:选取2021年7月—2022年6月我院收治的60例子宫内膜息肉患者,按随机数字表法分为两组,各30例。对照组予以宫腔镜冷刀切除术治疗,观察组术后加用LNG-IUS治疗,随访6个月。比较两组临床疗效、子宫内膜厚度、月经情况、生活质量及复发率。结果:观察组总有效率高于对照组,差异有统计学意义(P<0.05);观察组术后子宫内膜厚度、PBAC评分为(4.87±0.89)mm、(53.69±5.58)分,低于对照组的(5.89±1.02)mm、(62.89±6.04)分,差异有统计学意义(P<0.05);观察组术后生理、心理、社会及环境领域评分为(85.56±6.22)分、(86.82±6.43)分、(90.13±4.41)分、(91.43±4.13)分,高于对照组的(76.43±6.11)分、(80.09±6.25)分、(83.65±6.05)分、(86.06±6.19)分,复发率低于对照组,有统计学差异(P<0.05)。结论:宫腔镜冷刀切除术后予以LNG-IUS可提高子宫内膜息肉疗效,加快子宫内膜复常,缓解月经症状,且能减少疾病复发。

Objective:To investigate the clinical effect of levonorgestrel-releasing intrauterine system(LNG-IUS)in the treatment of endometrial polyps after hysteroscopic cold knife resection.Methods:60 patients with endometrial polyps admitted to our hospital from July 2021 to June 2022 were randomly divided into two groups,30 patients in each group.The control group was treated with hysteroscopic cold knife resection,and the observation group was treated with LNG-IUS after operation,with a follow-up of 6 months.The clinical efficacy,endometrial thickness,menstrual status,quality of life and recurrence rate were compared between the two groups.Results:The total effective rate of the observation group was significantly higher than that of the control group(P<0.05);The postoperative endometrial thickness and PBAC score of the observation group were(4.87±0.89)mm and(53.69±5.58)points,which were significantly lower than those of the control group(5.89±1.02)mm and(62.89±6.04)points(P<0.05);Postoperative scores in the fields of physiology,psychology,society and environment in the observation group were(85.56±6.22),(86.82±6.43),(90.13±4.41)and(91.43±4.13),which were higher than those in the control group(76.43±6.11),(80.09±6.25),(83.65±6.05)and(86.06±6.19),and the recurrence rate was lower than that in the control group(P<0.05).Conclusion:LNG-IUS after hysteroscopic cold knife resection can improve the efficacy of endometrial polyps,accelerate endometrial normalization,relieve menstrual symptoms,and reduce disease recurrence.
