急性脑卒中患者PTH、IL-6表达水平与其神经功能、近期预后的关系

Relationship between expression levels of PTH and IL⁃6 and neurological function and short⁃term prognosis in patients with acute stroke

目的分析急性脑卒中患者血清甲状旁腺素(PTH)、白细胞介素6(IL-6)表达水平与其神经功能、近期预后的关系。方法收集2019年1月至2022年1月郑州市第七人民医院收治的124例急性缺血性脑卒中(AIS)患者临床资料(AIS组),并纳入62名同期健康体检者为对照组,比较两组血清PTH、IL-6、25羟维生素D[25-(OH)D]及收缩压(SBP)、舒张压(DBP)差异,分析AIS患者血清PTH、IL-6与25-(OH)D、SBP、DBP的相关性,及入院时美国国立卫生研究院卒中量表(NIHSS)评分、随访3个月时改良Rankin量表(mRS)评分与血清PTH、IL-6的相关性。结果AIS组血清PTH、IL-6及SBP、DBP均显著高于对照组,血清25-(OH)D则低于对照组,差异均有统计学意义(P<0.05)。Pearson相关系数分析显示,AIS患者血清PTH、IL-6均与血清25-(OH)D呈显著负相关,均与SBP、DBP呈显著正相关(P<0.05)。NIHSS 16~45分者血清PTH、IL-6水平明显高于0~15分者,差异有统计学意义(P<0.05)。mRS评分3~5分者血清PTH、IL-6水平明显高于0~2分者,差异有统计学意义(P<0.05)。Pearson相关系数分析显示,AIS患者血清PTH、IL-6与入院时NIHSS评分及随访3个月mRS评分均呈显著正相关(P<0.05)。结论血清PTH、IL-6水平与AIS患者入院时神经功能及近期预后密切相关,有望成为AIS诊疗新靶点。

Objective To analyze the relationship of expression levels of serum parathyroid hormone(PTH)and interleukin⁃6(IL⁃6)with neurological function and short⁃term prognosis in patients with acute stroke.Methods The clinical data of 124 patients with acute ischemic stroke(AIS)who were treated in the Zhengzhou Seventh People's Hospital from January 2019 to January 2022 were collected(AIS group),and 62 healthy subjects with physical examination were included as the control group.The differences in serum PTH,IL⁃6,25⁃hydroxyvitamin D[25⁃(OH)D],systolic blood pressure(SBP)and diastolic blood pressure(DBP)were compared between the two groups,the correlation between serum PTH,IL⁃6 and 25⁃(OH)D,SBP,DBP in patients with AIS,and the correlation between National Institutes of Health Stroke Scale(NIHSS)score at admission,Modified Rankin Scale(mRS)score at 3 months and serum PTH,IL⁃6 was analyzed.Results Serum PTH,IL⁃6,SBP and DBP in the AIS group were significantly higher than those in the control group(P<0.05)while the level of serum 25⁃(OH)D was significantly lower than that in the control group(P<0.05).Serum levels of PTH and IL⁃6 in patients with AIS were negatively correlated with serum 25⁃(OH)D(P<0.05),and were positively correlated with SBP and DBP(P<0.05).The levels of serum PTH and IL⁃6 in patients with NIHSS score of 16⁃45 points were significantly higher than those in patients with NIHSS score of 0⁃15 points(P<0.05),and serum levels of PTH and IL⁃6 in patients with mRS score of 3⁃5 points were significantly higher than those in patients with mRS score of 0⁃2 points(P<0.05).Pearson correlation coefficient analysis showed that serum PTH and IL⁃6 in patients with AIS were significantly positively correlated with NIHSS score at admission and mRS score at 3 months of follow⁃up(P<0.05).Conclusion Serum PTH and IL⁃6 levels are closely related to neurological function at admission and short⁃term prognosis in patients with AIS,which may become new targets for the diagnosis and treatment of AIS.
