膝关节镜清理术后辅助关节腔内注射PRP治疗对OA患者Lysholm膝关节评分、VAS评分及并发症的影响

Effect of intra⁃articular injection of PRP after knee arthroscopic debridement on Lysholm knee score,VAS score and complications in patients with OA

目的分析膝关节镜清理术后辅助关节腔内注射PRP治疗对骨关节炎(OA)患者Lysholm膝关节评分、VAS评分及并发症的影响。方法选取2020年7月至2022年5月阜阳市第五人民医院收治的OA患者83例,按照治疗方案不同分为对照组(膝关节镜清理术后辅助关节腔内注射玻璃酸钠)40例和观察组(膝关节镜清理术后辅助关节腔内注射PRP)43例。对比两组临床疗效、Lysholm膝关节评分、VAS评分、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)、骨钙素(BGP)、β胶原降解产物(β-CTX)、胰岛素样生长因子-1(IGF-1)及并发症发生率。结果观察组总疗效高于对照组,差异有统计学意义(P<0.05)。观察组Lysholm膝关节评分高于对照组,VAS评分低于对照组,差异有统计学意义(P<0.05)。观察组躯体功能、僵硬、疼痛评分均低于对照组,差异有统计学意义(P<0.05)。观察组BGP、β-CTX、IGF-1水平均显著低于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率占比低于对照组,差异有统计学意义(P<0.05)。结论OA患者应用膝关节镜清理术后辅助关节腔内注射PRP治疗疗效理想,可有效改善患者Lysholm膝关节评分、VAS评分,改善骨代谢指标,减少并发症发生率,值得临床大力推广应用。

Objective To analyze the effects of assisted intra⁃articular injection of PRP on Lysholm knee score,VAS score and complications in patients with osteoarthritis(OA)after knee arthroscopic debridement.Methods A total of 83 cases of OA patients admitted to Fuyang Fifth People's Hospital from July 2020 to May 2022 were selected and divided into the control group(assisted articular injection of sodium hyaluronate after arthroscopy)(40 cases)and the observation group(assisted articular injection of PRP after arthroscopy)(43 cases)according to different treatment plans.Clinical efficacy,Lysholm knee score,VAS score,Western Ontario and McMaster University osteoarthritis index(WOMAC),osteocalcin(BGP),beta collagen degradation products(β⁃CTX),insulin⁃like growth factor⁃1(IGF⁃1)and complication rate were compared between the two groups.Results The total curative effect in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The Lysholm knee score in the observation group was higher than that in the control group,and the VAS score in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The physical function,stiffness,and pain scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).The levels of BGP,β⁃CTX and IGF⁃1 in the observation group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).The incidence rate of complications in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion The curative effect of intra⁃articular injection of PRP in patients with OA after arthroscopic debridement is satisfactory.It can effectively improve the Lysholm knee joint score,VAS score and bone metabolism index,reduce the incidence of complications.It is worthy of clinical promotion and application.
