经鼻高流量湿化氧疗治疗对支气管扩张合并感染患者肺功能指标及血清IL-6、γ-干扰素、TNF-α的影响 被引量:3

摘要 目的分析经鼻高流量湿化氧疗治疗对支气管扩张合并感染患者肺功能指标及血清白细胞介素-6(IL-6)、γ-干扰素(IFN-γ)及肿瘤坏死因子-α(NF-α)水平的影响。方法将2019年6月至2022年7月于河北省老年病医院就诊的支气管扩张合并感染患者153例纳入研究,按照不同治疗方法分为对照组(常规氧疗,n=75)和观察组(经鼻高流量湿化氧疗,n=78),观察两组动脉血氧分压(PaO_(2))、动脉血氧饱和度(SaO_(2))、动脉血二氧化碳分压(PaCO_(2))水平、第1秒用力呼气容积(FEV1)、用力肺活量(FVC)及FEV1/FVC、氧中毒、呼吸抑制、鼻腔干燥、IL-6、IFN-γ及TNF-α,并进行对比分析。结果与治疗前相比,治疗后两组PaO_(2)、SaO_(2)水平均明显升高,PaCO_(2)水平明显降低,其中观察组更为显著,差异有统计学意义(P<0.05)。与治疗前相比,治疗后两组FEV1、FVC、FEV1/FVC值均明显增加,其中观察组更为显著,差异有统计学意义(P<0.05)。观察组并发症发生次数明显减少,其发生率低于对照组,差异有统计学意义(P<0.05)。与治疗前相比,治疗后两组IL-6、IFN-γ及TNF-α水平均明显下降,其中观察组更为显著,差异有统计学意义(P<0.05)。结论针对支气管扩张合并感染患者实施经鼻高流量湿化氧疗有较满意的临床疗效,能有效改善患者的肺功能,促进其临床症状好转,控制病情发展,改善预后。 Objective To analyze the effects of nasal high⁃flow humidification oxygen therapy on pulmonary function indicators and serum Interleukin⁃6(IL⁃6)andγ⁃interferon IFN⁃γ),tumor necrosis factor⁃α(TNF⁃α)in patients with bronchiectasis complicated with infection.Methods 153 patients with bronchiecta⁃sis complicated by infection who were treated in our hospital from June 2019 to July 2022 were enrolled in the study,and according to different treatment methods,the patients were divided into the control group(conventional oxygen therapy,n=75)and The observation group(by nasal high⁃flow humidification oxygen therapy,n=78).Arterial partial pressure of oxygen(PaO_(2)),arterial oxygen saturation(SaO_(2)),arterial partial pres⁃sure of carbon dioxide(PaCO_(2)),forced expiratory volume in the first second(FEV1),forced vital capacity(FVC)and FEV1/FVC,oxygen poisoning,respiratory depression,dry nasal cavity,IL⁃6,IFN⁃γand TNF⁃αwere compared between the two groups.Results Compared with before treatment,the PaO_(2) and SaO_(2) levels in the two groups were significantly increased after treatment,and the PaCO_(2) level was significantly de⁃creased,especially in the observation group(P<0.05).Compared with before treatment,the values of FEV1,FVC and FEV1/FVC in both groups were significantly increased,especially in the observation group(P<0.05).The incidence of complications in the observation group was significantly lower than that in the control group(P<0.05).Compared with before treatment,the levels of IL⁃6,IFN⁃γand TNF⁃αin the two groups were significantly decreased,especially in the observation group(P<0.05).Conclusion Nasal high⁃flow humidified oxygen therapy for patients with bronchiectasis and infection has a satisfactory clinical effect,can effectively improve the lung function of the patients,promote the improvement of their clinical symptoms,control the development of the disease,and improve the prognosis.
