正常甲状腺功能人群TSH、FT3/FT4比值与代谢综合征的相关性研究

摘要 目的探讨正常范围的促甲状腺激素(TSH)和游离三碘甲状腺原氨酸/游离甲状腺素比率(FT3/FT4)与代谢综合征(MS)的相关性。方法7983例甲状腺功能正常员工纳入本研究,分别根据男女的TSH、FT3/FT4四分位数水平进行分组。结果随着TSH四分位数组别增高,男性腰围和甘油三酯呈升高趋势,HDL-C呈下降趋势;女性收缩压、舒张压和甘油三酯呈升高趋势,HDL-C呈下降趋势;随着FT3/FT4四分位数组别增高,男性收缩压、舒张压、腰围和甘油三酯均呈升高趋势,HDL-C呈下降趋势;女性舒张压和甘油三酯呈升高趋势,HDL-C呈下降趋势(P_(趋势值)<0.05)。校准年龄和BMI后,TSH与MS患病风险无显著相关(男性:P_(趋势值)=0.563,女性:P_(趋势值)=0.796),而FT3/FT4四分位数水平与MS患病风险呈正相关(男女P趋势值<0.001)。结论在甲状腺功能正常人群中,较高的FT3/FT4与MS患病风险升高有关,而TSH与MS无显著相关性。 Objective To explore the relationship between thyroid stimulating hormone(TSH)level and free triiodothyronine/free thyroxine ratio(FT3/FT4)with metabolic syndrome(MS)in an euthyroid cohort.Methods A total of 7983 euthyroid employees were included in this study,they were grouped by gender and TSH,FT3/FT4 quartile levels.Results As the TSH quartile increased,waist circumference and triglycerides tended to increase and HDL-C tended to decrease in males.The systolic blood pressure,diastolic blood pressure and triglycerides tended to increase and HDL-C tended to decrease in females.As the FT3/FT4 quartile increased,systolic blood pressure,diastolic blood pressure,waist circumference and triglycerides tended to increase and HDL-C tended to decrease in males.diastolic blood pressure and triglycerides tended to increase and HDL-C tended to decrease in females,with statistical significance(all P_(trend)<0.05).After adjustment for age and BMI,TSH was not significantly associated with the risk of MS(males:P_(trend)=0.563,females:P_(trend)=0.796),whereas the FT3/FT4 quartile was positively associated with the risk of metabolic syndrome(P_(trend)<0.001 for both genders).Conclusion In this euthyroid employees-cohort,higher FT3/FT4 ratio was associated with a higher risk of metabolic syndrome,whereas TSH was not significantly associated with MS.
