老年恶性淋巴瘤患者生存时间影响因素分析

目的探讨老年恶性淋巴瘤患者生存时间的影响因素,通过对相关影响因素进行综合分析、初步预测患者的生存时间,提前做好相关因素的预防,改良患者的治疗方案,延长老年恶性淋巴瘤患者的生存时间。方法选取2019年1月至2021年8月在本院血液科住院治疗的老年恶性淋巴瘤患者92例。收集老年恶性淋巴瘤患者初发疾病时的日常生活能力评分(ADL评分)、营养筛查评分(N-2)、查尔森并发症评分(CCI评分)等情况。研究老年恶性淋巴瘤患者生存时间与ADL评分、营养筛查评分、查尔森并发症评分之间的关系。结果利用COX回归模型作分析,在单因素分析中显示CCI评分和ADL评分影响患者生存时间,在多因素分析中显示CCI评分是患者生存时间的独立危险因素,同时将患者CCI评分与患者生存时间进行相关性分析,结果显示两者负相关,对CCI评分与患者生存时间进行K-M分析,结果表明患者存在合并症多,生存时间越短。结论临床工作中,需时刻评估老年恶性淋巴瘤患者的日常生活能力,全面掌握患者自身合并症情况,采用改善老年患者营养状况的治疗方法及护理干预,改善日常生活能力,积极治疗老年恶性淋巴瘤患者自身存在的合并症,逐步延长老年恶性淋巴瘤患者生存时间。

Objective To study the influencing factors of the survival time of elderly malignant lymphoma patients,through comprehensive analysis of relevant influencing factors,preliminarily judge the survival time of patients,to prevent relevant factors in advance,to improve the treatment plan of patients,and to prolong the survival time of elderly malignant lymphoma patients.Methods Retrospective research methods were used to analyze the elderly patients aged 60 years and above who were admitted to the Hematology Department of our hospital and diagnosed with malignant lymphoma from 2019 to 2021.The ability of daily living score(ADL score),nutrition screening score(N-2)and Charlson complication score(CCI score)of the elderly patients with malignant lymphoma were collected at the time of initial disease.The relationship between the survival time and ADL score,nutritional screening score and Charlson complication score in elderly patients with malignant lymphoma was analyzed.Results The Cox regression model was used for analysis.In the univariate analysis,it was shown that CCI score and ADL score affected the survival time of patients.In the multivariate analysis,it was shown that CCI score was an independent risk factor for the survival time of patients.At the same time,the correlation analysis between CCI score and survival time of patients showed that the two were negatively correlated.The kaplan-mare(K-M)analysis of CCI score and survival time of patients showed that patients had many comorbidities and shorter survival time.Conclusion In clinical work,it is necessary to evaluate the daily living ability of elderly malignant lymphoma patients at all times,comprehensively grasp their own comorbidities,adopt treatment methods and nursing interventions to improve their nutritional status,improve their daily living ability,actively treat their own comorbidities,and gradually prolong the survival time of elderly malignant lymphoma patients.
