新Bobath技术结合针刺治疗脑卒中后上肢痉挛的效果研究

目的:研究新Bobath技术结合针刺治疗脑卒中后上肢痉挛的临床疗效。方法:将广东省中西医结合医院康复科2022年1月至10月收治的脑卒中后上肢痉挛患者60例作为研究对象,以随机数表法分组,每组各30例。讨论组采用新Bobath技术结合针刺治疗,常模组单纯采用针刺治疗。比较两组的临床疗效及治疗前后的患肢运动功能和生活自理能力。结果:讨论组的临床总有效率高于常模组,差异有统计学意义(P<0.05)。两组治疗前的Fugl-Meyer评分相比,差异无统计学意义(P>0.05)。两组治疗后的Fugl-Meyer评分均高于治疗前,且讨论组治疗后的Fugl-Meyer评分高于常模组,差异有统计学意义(P<0.05)。两组治疗前的改良Barthel指数评分相比,差异无统计学意义(P>0.05)。两组治疗后的改良Barthel指数评分均高于治疗前,且讨论组治疗后的改良Barthel指数评分高于常模组,差异有统计学意义(P<0.05)。结论:对于脑卒中后上肢痉挛患者,临床采取新Bobath技术结合针刺的方法治疗能够获得理想疗效,有利于改善患者患肢的运动功能,提高生活自理能力,有临床借鉴意义和推广价值。

Objective:To study the clinical effect of new Bobath technology combined with acupuncture in the treatment of upper limb spasm after stroke.Methods:A total of 60 patients with upper limb spasm after stroke admitted to the Rehabilitation Department of Guangdong Hospital of Integrated Traditional Chinese and Western Medicine from January to October 2022 were selected as research objects.They were divided into 30 patients in each group by random number table method.The discussion group was treated with the new Bobath technique combined with acupuncture,while the norm group was treated with acupuncture alone.The clinical efficacy,motor function and self-care ability of affected limb before and after treatment were compared between the two groups.Results:The total clinical effective rate of the discussion group was higher than that of the normal group,and the difference was statistically significant(P<0.05).There was no significant difference in Fugl-Meyer scores between the two groups before treatment(P>0.05).The Fugl-Meyer score of the two groups after treatment was higher than that before treatment,and the Fugl-Meyer score of the discussion group after treatment was higher than that of the normal group,with statistical significance(P<0.05).There was no significant difference in modified Barthel index score between the two groups before treatment(P>0.05).The modified Barthel index score after treatment was higher in both groups than before treatment,and the modified Barthel index score after treatment in the discussion group was higher than that in the normal group,the difference was statistically significant(P<0.05).Conclusion:For the patients with upper limb spasm after stroke,the new Bobath technology combined with acupuncture can achieve ideal therapeutic effect,which is conducive to improving the motor function of the affected limb and improving the self-care ability of life,and has clinical reference significance and promotion value.
