卡波姆凝胶辅助重组人表皮生长因子滴眼液治疗白内障术后干眼症39例疗效分析

Efficacy Analysis of Carbomer Gel Assisted rhEGF Eye Drops in Patients with Xerophthalmia after Cataract Surgery

目的探究卡波姆(CBM)凝胶辅助重组人表皮生长因子(rhEGF)滴眼液治疗白内障术后干眼症患者的效果。方法选取内黄县人民医院2021年1月至2022年1月收治的78例白内障手术治疗后干眼症患者,按随机数字表法分成A组(n=39)、B组(n=39)。B组接受rhEGF滴眼液治疗,A组于B组基础上配以CBM凝胶辅助治疗。对比两组总有效率,治疗前后泪膜稳定性[泪膜破裂时间(BUT)、角膜荧光素染色(CFS)评分、基础泪液分泌试验Ⅰ(SIT)],症状积分,泪液动力学指标[泪新月总面积(TTMA)平均值],肿瘤坏死因子-α(TNF-α),白细胞介素-1β(IL-1β),IL-2水平。结果A组总有效率97.44%(38/39)较B组79.49%(31/39)高(P<0.05);治疗30 d后A组BUT较B组高,SIT较B组长,CFS评分较B组低(P<0.05);治疗30d后A组异物感、疲劳感、干燥感、视力波动症状积分较B组低(P<0.05);最后一次用药后10 min、30 min、60 min A组TTMA平均值较B组高(P<0.05);治疗30 d后A组泪液IL-1β、IL-2、TNF-α水平较B组低(P<0.05)。结论CBM凝胶辅助rhEGF滴眼液治疗白内障术后干眼症患者可进一步提升治疗效果及泪膜稳定性,改善泪液动力学水平,缓解炎性反应,促进病情恢复。

Objective To investigate the effect of carbomer(CBM)gel assisted recombinant human epidermal growth factor(rhEGF)eye drops in the treatment of patients with xerophthalmia after cataract surgery.Methods A total of 78 patients with xerophthalmia after cataract surgery in Neihuang County People's Hospital from January 2021 to January 2022 were selected and divided into group A(n=39)and group B(n=39)according to the random number table method.Group B received rhEGF eye drops,and group A received CBM gel adjuvant therapy on the basis of group B.The total effective rate,tear film stability[tear film break-up time(BUT),corneal fluorescein staining(CFS)score,basal tear secretion test I(SIT)],symptom scores,and tear dynamics indexes[total crescent area(TTMA)mean],tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),IL-2 levels before and after treatment were compared between the two groups.Results The total effective rate in group A was 97.44%(38/39)higher than 79.49%(31/39)in group B(P<0.05).After 30 days of treatment,the BUT of group A was shorter than that of group B,the SIT of group A was longer than that of group B,and the CFS score was higher than that of group B(P<0.05).After 30 days of treatment,the symptom scores of foreign body sensation,fatigue,dryness and visual fluctuation in group A were lower than those in group B(P<0.05);the mean TTMA of group A was higher than that of group B 10 min,30 min and 60 min after the last administration(P<0.05);tear IL-1 in group A after 30 days of treatmentβ、IL-2、TNF-αlevels was lower than that of group B(P<0.05).Conclusion CBM gel assisted with rhEGF eye drops in the treatment of xerophthalmia patients after cataract surgery can further improve the therapeutic effect and tear film stability,improve tear dynamics,relieve inflammatory response,and promote disease recovery.
