摘要
目的探讨心脏超声智能导航技术(FINE,5D Heart)在产前诊断圆锥动脉干畸形胎儿中的临床价值。方法选择2019年7月—2021年7月在该院妇产科接受产前诊断的73例中晚孕期单胎孕妇为研究对象,均接受心脏超声检查,并运用5D Heart分析软件对超声图像实施处理。并以产后超声随访为金标准,对比2D超声与5D Heart分析软件诊断准确率、5D Heart分析软件处理前后各平面显示率以及各诊断要素图像质量评分与诊断耗时指标。结果5D Heart分析软件诊断CTD胎儿的准确率为89.74%,明显高于2D超声诊断的69.23%,差异有统计学意义(P<0.05);利用2D超声,73例孕妇共获得136个容积数据,其中有效容积121个,获取容积的成功率为88.97%;观察者A应用5D Heart分析软件处理后与常规超声诊断及金标准一致(均P>0.05),观察者B应用5D Heart分析软件处理后获得大致相等分数(P<0.05),但诊断效能不及常规超声(均P<0.05)和金标准(均P<0.05)。结论5D Heart分析软件诊断操作简单方便,对机体损伤较小,可重复性高,对胎儿圆锥动脉干畸形诊断准确率高,并且可缩短诊断时间,提高临床医师工作效率,值得临床广泛推广应用。
Objective To evaluate the clinical value of cardiac ultrasound intelligent navigation technology(FINE,5D heart)in prenatal diagnosis of fetal conus trunk malformation.Methods From July 2019 to July 2021,73 singleton pregnant women in middle and late pregnancy who received prenatal diagnosis in the department of Obstetrics and Gynecology of the hospital were selected as the research subjects.All of them received cardiac ultrasound examination,and the ultrasound images were processed by 5D Heart analysis software.With postpartum ultrasound follow-up as the gold standard,the diagnostic accuracy of 2D ultrasound and 5D Heart analysis software,the display rate of each plane before and after 5D Heart analysis software,as well as the image quality score of each diagnostic factor and diagnostic time index were compared.Results The accuracy of 5D heart analysis software in diagnosing CTD fetus was 89.74%,which was significantly higher than 69.23%in 2D ultrasound diagnosis(P<0.05);Using 2D ultrasound,a total of 136 volume data were obtained from 73 pregnant women,including 121 effective volumes,and the success rate of obtaining volume was 88.97%;Observer A treated with 5D Heart analysis software was consistent with conventional ultrasound diagnosis and gold standard(all P>0.05),and observer B treated with 5D Heart analysis software received roughly equal scores(P<0.05),but the diagnostic efficacy was not as good as conventional ultrasound(allP<0.05)and gold standard(allP<0.05).Conclusion 5D Heart analysis software is simple and convenient to diagnose,has little damage to the body,high repeatability,and high accuracy in the diagnosis of fetal conus artery malformation.It can shorten the diagnosis time and improve the work efficiency of clinicians,which is worthy of widespread clinical application.
