老年人牙周炎与睡眠焦虑抑郁的相关性研究

Study on the Correlation Between Periodontitis and Sleep,Anxiety and Depression in the Elderly

目的:探讨老年人牙周炎与睡眠、焦虑、抑郁的相关性。方法:选取2020年2月至2021年到厦门大学附属第一医院口腔科就诊的120例老年人作为研究对象,回顾分析其病历资料,采用匹兹堡睡眠指数量表(PSQI)、健康问卷抑郁量表(PHQ-9)、广泛性焦虑障碍量表(GAD-7)评估患者睡眠质量、焦虑和抑郁状况,并根据患者是否存在牙周炎疾病分为牙周炎组、非牙周炎组,单因素、Logistic回归分析PSQI评分、PHQ-9评分、GAD-7评分与老年人牙周炎的相关性。结果:牙周炎组性别占比、学历程度、BMI指数与牙周健康组比较,差异均无统计学意义(均P>0.05);牙周炎组PSQI评分16~21分率50.00%、GAD-7评分15~21分率62.07%、GAD-7评分10~14分率25.86%、PHQ-9评分20~27分率46.55%高于牙周健康组PSQI评分16~21分率11.29%、GAD-7评分15~21分率17.74%、GAD-7评分10~14分率16.13%、PHQ-9评分20~27分率9.68%,2组比较差异均有统计学意义(均P<0.05);Logistic回归分析结果显示,PSQI评分(OR为0.821、SE为0.026、OR95%CI为0.112~0.968)、PHQ-9评分(OR为1.021、SE为0.031、OR95%CI为0.351~1.698)、GAD-7评分(OR为0.514、SE为0.002、OR95%0.253~1.221)为影响老年人牙周炎与睡眠、焦虑、抑郁相关因素(P<0.05)。结论:老年人牙周炎会对其睡眠质量、自身情绪造成影响,同时,睡眠质量差、抑郁焦虑情绪也会影响到牙周炎病情,提示身心健康与口腔健康之间有正性关联。

Objective:To explore the correlation between periodontitis and sleep,anxiety and depression in the elderly.Methods:A total of 120 elderly patients who came to the Department of Stomatology from February 2020 to 2021 were selected as the research objects,and their medical records were retrospectively analyzed.The sleep status,anxiety and depression status of the patients were evaluated with Pittsburgh sleep index scale(PSQI),health questionnaire Depression Scale(PHQ-9),and generalized anxiety disorder scale(GAD-7).According to whether the patients had periodontitis,they were divided into periodontitis group,non periodontitis group,univariate Logistic regression was used to analyze the correlation between PSQI score,PHQ-9 score,GAD-7 score and periodontitis in the elderly.Results:There was no significant difference between periodontitis group and periodontal healthy group in sex ratio,education level and BMI index(P>0.05);The PSQI score of 16~21,50%;GAD~7 score of 15~21,62.07%;GAD-7 score of 10~14,25.86%,PHQ-9 score of 20~27,46.55%in periodontitis group were higher than those in periodontal healthy group(11.29%,17.74%,16.13%,9.68%),the difference between the two groups was statistically significant P<0.05;Logistic regression analysis showed that PSQI score(OR is 0.821,SE is 0.026,OR 95%CI is 0.112 to 0.968),PHQ-9 score(OR is 1.021,SE is0.031,OR 95%CI is 0.351 to 1.698),GAD-7 score(OR is,0.514,SE is 0.002,OR 95%is 0.253 to 1.221)were related to effect on sleep,anxiety and depression(P<0.05)to elderly with periodontitis.Conclusion:Periodontitis in the elderly will affect their sleep quality and their own emotions.At the same time,poor sleep quality,depression and anxiety will also affect the condition of periodontitis,suggesting that there is a positive correlation between physical and mental health and oral health.
