保乳术与改良根治术治疗乳腺癌患者的效果对比

Comparison Efficacy of Breast Conserving Surgery and Modified Radical Mastectomy in Patients with Breast Cancer

目的:探究保乳术与改良根治术治疗乳腺癌患者的效果对比。方法:回顾性分析2019年4月-2021年5月景德镇市妇幼保健院收治的60例乳腺癌患者的临床资料。依据手术方法不同分为A组(行保乳术治疗)38例及B组(行改良根治术)22例。对比两组围手术期指标、美容效果、生活质量评分及并发症发生率。结果:A组手术时间、住院时间分别为(109.86±23.61)min、(7.46±0.58)d,均短于B组的(168.49±20.57)min、(14.49±2.34)d;术中出血量、术后引流量分别为(210.35±20.16)、(402.31±15.29)mL,均少于B组的(297.65±15.83)、(584.19±16.37)mL,差异均有统计学意义(P<0.05)。A组乳房美容优良率、一般率均高于B组,差的比例低于B组,差异均有统计学意义(P<0.05)。术后3个月,A组生活质量评分为(97.58±5.49)分,高于B组的(87.31±5.38)分,差异有统计学意义(P<0.05)。A组并发症发生率为7.89%,低于B组的36.36%,差异有统计学意义(P<0.05)。结论:保乳术与改良根治术均可有效治疗乳腺癌,但保乳术手术效果更佳,术后乳房美容效果更好,有助于提升术后生活质量,且并发症发生率低,安全性高,值得推广应用。

Objective:To explore the effect of breast conserving surgery and modified radical mastectomy in the treatment of breast cancer patients.Method:The clinical data of 60 patients with breast cancer admitted to Jingdezhen Maternal and Child Health Hospital from April 2019 to May 2021 were retrospectively analyzed.According to different surgical methods,they were divided into group A(38 cases underwent breast conserving surgery)and group B(22 cases underwent modified radical mastectomy).The perioperative indicators,cosmetic effects,quality of life scores and incidence of complications were compared between the two groups.Result:The operation time and hospital stay of the group A were(109.86±23.61)min and(7.46±0.58)d,respectively,which were shorter than(168.49±20.57)min and(14.49±2.34)d of the group B;intraoperative blood loss and postoperative drainage volume were(210.35±20.16)and(402.31±15.29)mL,respectively,which were lower than(297.65±15.83)and(584.19±16.37)mL of the group B,the differences were statistically significant(P<0.05).The excellent and good rate and general rate of breast beauty of group A were higher than those of the group B,and the proportion of poor was lower than that of the group B,the differences were statistically significant(P<0.05).3 months after surgery,the life quality score of the group A was(97.58±5.49)points,which was higher than(87.31±5.38)points of the group B,the difference was statistically significant(P<0.05).The incidence of complications of the group A was 7.89%,which was lower than 36.36%of the group B,the difference was statistically significant(P<0.05).Conclusion:Both breast conserving surgery and modified radical mastectomy are effective in the treatment of breast cancer,but breast conserving surgery has better surgical effect,better postoperative breast cosmetic effect,help to improve postoperative quality of life,and the incidence of complications is low,high safety,worthy of promotion and application.
