解剖锁定钢板内固定治疗不同分型肱骨近端骨折的效果及对肩关节功能恢复的影响

Effect of Anatomical Locking Plate Internal Fixation on Different Types of Proximal Humerus Fracture and Its Influence on Shoulder Joint Function Recovery

目的:探讨在不同分型肱骨近端骨折患者中实施解剖锁定钢板内固定治疗的效果及对肩关节功能恢复的影响。方法:选取2020年1月-2021年12月于九江市柴桑区人民医院治疗的肱骨近端骨折患者100例作为研究对象,根据Neer分型将其分为A组(NeerⅡ)、B组(NeerⅢ)、C组(NeerⅣ),三组患者均实施解剖锁定钢板内固定治疗。对比三组围手术期相关指标及治疗前后的肩关节活动度、Constant-Murley肩关节功能评分。结果:A组肩关节外旋、前屈上举、外展的活动度及Constant-Murley肩关节功能评分均高于B组、C组,且B组均高于C组(P<0.05);A组术中出血量、手术时间及骨折愈合时间均优B组和C组,且B组均优于C组(P<0.05)。结论:解剖锁定钢板内固定治疗肱骨近端骨折的临床效果较好,可提高患者的肩关节功能,且Neer分型越低,效果越明显。

Objective:To investigate the effect of anatomical locking plate internal fixation in patients with different types of proximal humerus fractures and its influence on shoulder joint functional recovery.Method:A total of 100 patients with proximal humerus fracture who were treated in Jiujiang Chaisang District People's Hospital from January 2020 to December 2021 were selected as the research subjects and divided into group A(NeerⅡ),group B(NeerⅢ)and group C(NeerⅣ)according to Neer classification.All the patients in the three groups received anatomical locking plate internal fixation.The perioperative related indicators and before and after treatment of the shoulder joint range of motion and Constant-Murley shoulder function score were compared among the three groups.Result:The range of motion of shoulder joint external rotation,anterior flexion lifting,abduction and Constant-Murley shoulder function scores in group A were higher than those in group B and group C,and those in group B were higher than those in group C(P<0.05).The Intraoperative bleeding volume,operation time and fracture healing time in group A were better than those in group B and group C,and group B was better than those in group C(P<0.05).Conclusion:The clinical effect of anatomical locking plate internal fixation in the treatment of proximal humeral fractures is good,which can improve the shoulder joint function of patients,and the lower Neer classification,the more obvious the effect.
