彩色多普勒血流成像对鼻咽癌放疗后椎动脉狭窄的诊断价值分析

Diagnostic value of color Doppler flow imaging in vertebral artery stenosis after radiotherapy for nasopharyngeal carcinoma

目的探讨彩色多普勒血流成像对鼻咽癌放疗后椎动脉狭窄的诊断价值。方法择取2016年1月—2020年1月,进行放疗同时生存期>2年的鼻咽癌患者(n=15),作为研究组,同时将初治尚未进行放疗的鼻咽癌患者(n=15),作为对照组。统计两组颈动脉狭窄情况,记录血管内径、收缩期峰值流速、舒张末期血流速度。结果对照组放疗前后颈总动脉、颈内动脉、颈外动脉狭窄比较,差异无统计学意义(P>0.05);内径<15.00%分析中,研究组颈总动脉、颈内动脉狭窄度高于对照组(P<0.05);颈外动脉比较,两组比较形成差异性(P<0.05);颈动脉内径>50.00%分析中,两组颈总动脉、颈内动脉有显著差异(P<0.05);颈外动脉内径比较不具有统计学意义(P>0.05)。结论行放疗处理后的鼻咽癌患者,放疗后颈动脉狭窄相对多见,狭窄多发于颈总动脉及颈内动脉。

Objective To investigate the value of color Doppler flow imaging in the diagnosis of vertebral artery stenosis after radiotherapy for nasopharyngeal carcinoma.Methods Patients with nasopharyngeal carcinoma(n=15)who received radiotherapy and had a survival time of more than 2 years from January 2016 to January 2020 were selected as the study group,and patients with nasopharyngeal carcinoma who had not received radiotherapy for the first time(n=15)were selected as the control group.The carotid stenosis of the two groups was counted,and the vessel diameter,peak systolic velocity and end diastolic velocity were recorded.Results There was no significant difference in the stenosis of common carotid artery,internal carotid artery and external carotid artery before and after radiotherapy in the control group(P>0.05);In the analysis of inner diameter<15.00%,the stenosis of common carotid artery and internal carotid artery in the study group was higher than that in the control group(P<0.05);There was significant difference between the two groups(P<0.05);In the analysis of carotid artery diameter>50.00%,there was significant difference between the two groups in common carotid artery and internal carotid artery(P<0.05);The comparison of external carotid artery diameter was not statistically significant(P>0.05).Conclusion In patients with nasopharyngeal carcinoma after radiotherapy,carotid artery stenosis is relatively common,and stenosis mostly occurs in common carotid artery and internal carotid artery.
