右美托咪定在全麻鼻内镜手术中的应用及对患者心率的影响

Application of dexmedetomidine in endoscopic sinus surgery under general anesthesia and its effect on heart rate of patients

目的研究全麻鼻内镜手术患者应用右美托咪定的效果。方法数据取自本院2020年1月-2021年12月收治的60例鼻内镜手术患者,“随机取样法”分基础组(静脉滴注滴注等量生理盐水,n=30)、研讨组(右美托咪定静脉滴注给药,n=30),2组血流动力学、临床指标及麻醉效果比较分析。结果麻醉前比较2组血流动力学无差异,P>0.05;麻醉后与基础组比较,研讨组HR、MAP、SBP及DBP指标更低;术中出血量更少,麻醉达标、拔管时呛咳及拔管时间更短;研讨组有效率(96.67%)高于基础组(76.67%),χ^(2)=5.192,P=0.023,P<0.05(具有统计学意义)。结论右美托咪定静脉给药用于全麻鼻内镜手术中可改善血流动力学、控制血压水平,减少术中出血量、提高麻醉效果,值得推崇。

Objective To study the effect of dexmedetomidine in patients undergoing nasal endoscopic surgery under general anesthesia.Methods The data were collected from 60 patients with nasal endoscopic surgery under general anesthesia who were treated in our hospital from January 2020 to December 2021.The“random sampling method”was divided into the basic group(nasal instillation of the same amount of normal saline,n=30),In the research group(dexmedetomidine intranasal administration,n=30),the hemodynamics,clinical indicators and anesthesia effects of the two groups were compared and analyzed.Results There was no difference in hemodynamics between the two groups before anesthesia,P>0.05;after anesthesia,compared with the basic group,the HR,MAP,SBP and DBP indexes of the research group were lower;intraoperative blood loss was less,the anesthesia was up to standard,and the catheter was extubated.The time of choking and extubation was shorter;the effective rate of the research group(96.67%)was higher than that of the basic group(76.67%),χ^(2)=5.192,P=0.023,P<0.05(statistically significant).Conclusion Dexmedetomidine intranasal administration can improve hemodynamics,control blood pressure,reduce intraoperative blood loss and improve anesthesia effect in endoscopic sinus surgery under general anesthesia,which is worthy of praise.
