不孕症体外受精-胚胎移植反复种植失败患者心理困扰对妊娠结局的影响

Influence of Psychological Distress on the Pregnancy Outcome of Infertile Patients with Repeated Implantation Failure of in Vitro Fertilization Embryo Transfer

目的探讨不孕症体外受精-胚胎移植(IVF-ET)反复种植失败患者心理困扰对其妊娠结局的影响。方法前瞻性纳入2021年1月至2022年1月河南省人民医院生殖中心收治的80例不孕症患者作为研究对象,设计一般资料收集表,填写并记录患者的相关临床资料,采用凯斯勒心理困扰量表(K10)评估患者的心理困扰情况,比较不同资料患者K10评分,分析其影响因素,并探讨患者心理困扰对其妊娠结局的影响。结果不孕症患者K10总评分为(31.48±2.69)分;不同年龄、社会支持、家庭关怀、知识了解程度、妊娠结局患者K10评分比较,差异有统计学意义(P<0.05);经多元线性回归分析,结果显示,年龄、社会支持、家庭关怀、知识了解程度是患者K10评分的影响因素(P<0.05);经logistic回归分析结果显示,心理困扰与患者的妊娠结局有关(P<0.05)。结论不孕症IVF-ET反复种植失败患者心理困扰处于中等水平,受年龄、社会支持、家庭关怀、知识了解程度的影响,且心理困扰会影响其妊娠结局。

Objective To explore the influence of psychological distress on the pregnancy outcome of infertile patients with repeated implantation failure of in vitro fertilization embryo transfer(IVF-ET).Methods A total of 80 infertility patients admitted to the Reproductive Center of Henan Provincial People’s Hospital from January 2021 to January 2022 were prospectively included as the study subjects.The general data collection form was designed,the relevant clinical data of the patients were filled in and recorded,and the psychological distress of the patients was assessed by the Kessler psychological distress scale(K10).The K10 score of patients with different baseline data was compared to analyze its influencing factors,and the influence of psychological distress on pregnancy outcome was discussed.Results The total K10 score of infertility patients was(31.48±2.69).There were statistical significant differences in K10 scores among patients with different ages,social support,family care,knowledge understanding and pregnancy outcomes(P<0.05).The results of multiple linear regression analysis showed that age,social support,family care and knowledge understanding were the influencing factors of patients’K10 score(P<0.05).Logistic regression analysis showed that psychological distress was related to the pregnancy outcome(P<0.05).Conclusion The psychological distress of infertile patients with repeated IVF-ET implantation failure is at a moderate level,which is affected by age,social support,family care and knowledge understanding,and will affect their pregnancy outcomes.
