长链非编码RNA HIF1A-AS1与老年脑卒中病情严重程度及预后的相关性分析

摘要 目的探讨老年脑卒中患者血清长链非编码RNA(LncRNA)HIF1A-AS1的表达及其与老年脑卒中患者病情严重程度及预后的关系。方法选取106例老年脑卒中患者(脑卒中组)及70名健康体检者(正常对照组)进行研究。采用实时荧光定量PCR(RT-qPCR)检测血清中LncRNA HIF1A-AS1表达。采用Pearson相关性分析HIF1A-AS1与NIHSS评分的相关性。COX比例风险回归模型分析影响老年脑卒中患者不良预后的因素,ROC曲线分析HIF1A-AS1对老年脑卒中不良预后的预测价值。结果与正常对照组相比,脑卒中组血清HIF1A-AS1表达显著升高(t=20.154,P=0.000)。与脑卒中组HIF1A-AS1低表达亚组比较,HIF1A-AS1高表达亚组高密度脂蛋白显著降低,NIHSS评分显著升高(均P<0.05)。随着老年脑卒中患者病情程度的增加,血清HIF1A-AS1表达逐渐升高,三组比较差异有统计学意义(F=38.461,P=0.000),两两比较差异亦有统计学意义(均P<0.05)。老年脑卒中患者血清HIF1A-AS1水平与NIHSS评分呈正相关(P<0.05)。预后不良组血清HIF1A-AS1表达显著高于预后良好组(t=8.462,P=0.000)。多因素分析显示,高密度脂蛋白、NIHSS评分、HIF1A-AS1表达为影响老年脑卒中患者预后的独立危险因素(均P<0.05)。ROC分析显示,HIF1A-AS1对老年脑卒中不良预后预测的曲线下面积为0.845(95%CI:0.762~0.908),最佳截断值为2.24,敏感度为71.14%,特异度为88.41%。结论老年脑卒中患者血清HIF1A-AS1中的表达升高,与患者病情严重程度及预后有关,对患者不良预后具有一定的预测价值。 Objective To investigate the expression of long non-coding RNA(LncRNA)HIF1A-AS1 in the serum of elderly stroke patients,and to analyze its relationship with the severity and prognosis of elderly stroke patients.Methods A total of 106 elderly stroke patients(stroke group)and 70 healthy people(normal control group)were selected for the study.The expression of LncRNA HIF1A-AS1 in serum was detected by real-time fluorescent quantitative PCR(RT-qPCR).Pearson correlation was used to analyze the correlation between HIF1A-AS1 and NIHSS.COX proportional hazards regression model was used to analyze the factors affecting the poor prognosis of elderly stroke patients,and ROC curve was used to analyze the predictive value of HIF1A-AS1 on the poor prognosis of elderly stroke patients.Results Compared with that in the normal control group,the expression of HIF1A-AS1 was significantly increased in the stroke group(t=20.154,P=0.000).Compared with those in the HIF1A-AS1 low expression subgroup of stroke group,the high-density lipoprotein in the HIF1A-AS1 high expression subgroup was significantly decreased and the NIHSS score was significantly increased(all P<0.05).With the increase of the severity of stroke in elderly patients,the expression of serum HIF1A-AS1 gradually increased,and the difference between the three groups was statistically significant(F=38.461,P=0.000),and the difference between the two groups was also statistically significant(all P<0.05).Serum HIF1A-AS1 level was positively correlated with NIHSS score in elderly patients with stroke(P<0.05).The expression of serum HIF1A-AS1 in the poor prognosis group was significantly higher than that in the good prognosis group(t=8.462,P=0.000).Multivariate analysis showed that high-density lipoprotein,NIHSS score and HIF1A-AS1 expression were independent risk factors affecting the prognosis of elderly stroke patients(all P<0.05).ROC curve analysis showed that the areas under curve of HIF1A-AS1 for predicting poor prognosis of elderly stroke was 0.845(95%CI:0.762-0.908),the best cutoff value was 2.24,the sensitivity was 71.14%,and the specificity was 88.41%.Conclusion The increased expression of serum HIF1A-AS1 in elderly stroke patients is related to the severity of the disease and the prognosis of the patients,and has a certain predictive value for the poor prognosis of patients.
