急性缺血性脑卒中患者血管内介入治疗后S100A12、高迁移率族蛋白B1表达及与继发性脑出血的相关性分析 被引量:7

摘要 目的探讨S100A12、高迁移率族蛋白B1(HMGB1)与急性缺血性脑卒中(AIS)血管内介入治疗后继发性脑出血的相关性。方法根据血管内介入治疗结束3 d后复查的头颅CT结果,将94例AIS患者分为继发性脑出血组和无继发性脑出血组。采用ELISA法检测AIS患者在治疗前12 h内及治疗后1周血清中S100A12、HMGB1水平。Pearson分析治疗后S100A12、HMGB1与超敏C反应蛋白(hs-CRP)、IL-6、TNF-α的相关性。Logistic回归分析影响AIS患者血管内介入治疗后继发脑出血危险因素。结果与治疗前相比,治疗后AIS患者血清S100A12、HMGB1、hs-CRP、IL-6、TNF-α水平均显著降低(均P<0.05)。与未继发脑出血组相比,继发脑出血组血清S100A12、HMGB1、hs-CRP、IL-6、TNF-α水平升高(均P<0.05)。Pearson分析显示,S100A12与hs-CRP、IL-6、TNF-α呈正相关(r=0.485,r=0.516,r=0.534;均P<0.001);HMGB1与hs-CRP、IL-6、TNF-α呈正相关(r=0.411,r=0.462,r=0.508;均P<0.001)。Logistic回归分析显示,高水平S100A12、高水平HMGB1是AIS患者血管内介入治疗后继发脑出血的危险因素(均P<0.05)。结论HMGB1、S100A12在AIS血管内介入治疗后继发性脑出血的患者中表达水平上调,与治疗后继发性脑出血关系密切。 Objective To explore the correlation between the expression of S100A12,high mobility group box protein 1(HMGB1)and secondary cerebral hemorrhage in acute ischemic stroke(AIS)patient after intravascular interventional therapy.Methods According to the results of head CT review 3 d after endovascular interventional therapy,94 AIS patients were divided into secondary cerebral hemorrhage group and non-secondary cerebral hemorrhage group.ELISA was used to detect the serum levels of S100A12 and HMGB1 in AIS patients within 12 h before treatment and 1 week after treatment.Pearson method was used to analyze the correlation between S100A12,HMGB1 and high-sensitivity C-reactive protein(hs-CRP),IL-6,TNF-αafter treatment.Logistic regression was used to analyze the risk factors affecting the secondary cerebral hemorrhage after intravascular interventional therapy in AIS patients.Results Compared with those before treatment,the serum S100A12,HMGB1,hs-CRP,IL-6 and TNF-αlevels in AIS patients were significantly lower after treatment(all P<0.05).Compared with those in the non-secondary intracerebral hemorrhage group,the serum S100A12,HMGB1,hs-CRP,IL-6 and TNF-αlevels in the secondary intracerebral hemorrhage group were significantly higher(all P<0.05).Pearson analysis showed that S100A12 was positively correlated with hs-CRP,IL-6 and TNF-α(r=0.485,r=0.516,r=0.534;all P<0.001);HMGB1 was positively correlated with hs-CRP,IL-6 and TNF-α(r=0.411,r=0.462,r=0.508;all P<0.001).Logistic analysis showed that high level of S100A12 and high level of HMGB1 were risk factors affecting the secondary cerebral hemorrhage in patients with AIS after intravascular interventional therapy(all P<0.05).Conclusion The expression levels of HMGB1 and S100A12 are up-regulated in patients with secondary cerebral hemorrhage after intravascular interventional therapy of AIS,and they are closely related to secondary cerebral hemorrhage after treatment.
