高电压长时程脉冲射频治疗对肩周炎患者肩关节功能及疼痛介质表达的影响

Influence of High-voltage Long-duration Pulsed Radiofrequency Treatment on Shoulder Joint Function and Expression of Pain Mediators in Patients with Scapulohumeral Periarthritis

目的:观察与分析高电压长时程脉冲射频治疗对肩周炎患者肩关节功能及疼痛介质表达的影响。方法:选取2018年5月-2021年1月就诊于北京中京苑医学科学研究院恒安中医院的72例肩周炎患者为研究对象,根据随机数字表法分为对照组和观察组,每组36例。对照组采用关节内注射治疗,观察组采用高电压长时程脉冲射频治疗。比较两组的临床总有效率、治疗前后的肩关节功能(Canstant-Murley评分)及疼痛介质[血清前列腺素E2(PGE2)、P物质(SP)及白介素-6(IL-6)]表达水平。结果:治疗后,观察组的临床总有效率显著高于对照组,差异有统计学意义(P<0.05)。治疗前,两组的Canstant-Murley各维度评分及疼痛介质表达水平比较,差异均无统计学意义(P>0.05);治疗后,两组的Canstant-Murley各维度评分均显著高于治疗前,且观察组均显著高于对照组,两组的疼痛介质表达水平均显著低于治疗前,且观察组均显著低于对照组,差异均有统计学意义(P<0.05)。结论:高电压长时程脉冲射频治疗肩周炎患者的临床效果较好,可显著改善肩关节功能及疼痛介质表达水平,在肩周炎患者中的应用价值较高。

Objective:To observe and analyze the influence of high-voltage long-duration pulsed radiofrequency treatment on shoulder joint function and expression of pain mediators in patients with scapulohumeral periarthritis.Method:A total of 72 patients with scapulohumeral periarthritis in Heng'an Hospital of Traditional Chinese Medicine,Zhongjingyuan Institute of Medical Sciences from May 2018 to January 2021 were selected as the research objects,they were divided into control group and observation group according to random number table method,with 36 cases in each group.The control group was treated with intraarticular injection,and the observation group was treated with high-voltage long-duration pulsed radiofrequency treatment.The total clinical effective rate,shoulder joint function(Canstant-Murley score)and expression levels of pain mediators[serum prostaglandin E 2(PGE 2),substance P(SP)and interleukin-6(IL-6)]before and after the treatment between two groups were compared.Result:After treatment,the total clinical effective rate of the observation group was significantly higher than that of control group,there was statistically significant difference(P<0.05).Before treatment,there were no significant differences in Canstant-Murley scores for each dimension and expression levels of pain mediators between the two groups(P>0.05);after treatment,Canstant-Murley scores of all dimensions in both groups were significantly higher than those before treatment,and the scores of observation group were significantly higher than those of control group,and serum pain mediators of two groups were significantly lower than those of control group,the mediators were significantly lower than those of control group,the differences were statistically significant(P<0.05).Conclusion:The clinical effect of high-voltage long-duration pulsed radiofrequency treatment in the patients with scapulohumeral periarthritis is better,which can significantly improve the shoulder joint function and expression level of pain mediators,so its application value in patients with scapulohumeral periarthritis is higher.
