糖尿病肾病维持性血液透析患者低血糖恐惧感现状及相关因素研究

Hypoglycemia Fear Current Situation and Related Factors in Diabetic Nephropathy Patients with Maintenance Hemodialysis

目的:探讨糖尿病肾病维持性血液透析(DN-MHD)患者低血糖恐惧感现状及相关因素。方法:用便利抽样法选取2020年2月-2022年3月在抚州市第一人民医院行MHD治疗的86例DN患者,分析DN-MHD患者低血糖恐惧感现状及相关因素。结果:DN-MHD患者低血糖恐惧感评分为(40.02±6.12)分。单因素分析显示,不同焦虑情绪、过去一个月低血糖发生次数、低血糖严重程度、透析年限DN-MHD患者的低血糖恐惧感评分比较,差异均有统计学意义(P<0.05)。不同年龄、婚姻状况、文化程度、性别、透析上机前血糖、居住方式、家庭人均月收入、体重指数(BMI)、糖尿病病程DN-MHD患者低血糖恐惧感评分比较,差异均无统计学意义(P>0.05)。多因素分析显示,过去一个月低血糖发生次数、焦虑情绪、低血糖严重程度、透析年限是DN-MHD患者低血糖恐惧感的影响因素(P<0.05)。结论:DN-MHD患者低血糖恐惧感偏高,其受过去一个月低血糖发生次数、焦虑情绪、低血糖严重程度、透析年限影响。

Objective:To investigate the hypoglycemia fear status situation and related factors in diabetic nephropathy patients with maintenance hemodialysis(DN-MHD).Method:A total of 86 DN patients with MHD in First People's Hospital of Fuzhou from February 2020 to March 2022 were selected by convenience sampling,and the current situation of hypoglycemia fear and related factors in DN-MHD patients were analyzed.Result:The hypoglycemic fear score of DN-MHD patients was(40.02±6.12)points.Univariate analysis showed that there were statistically significant differences in hypoglycemic fear scores among patients with different anxiety,frequency of hypoglycemia in the past month,severity of hypoglycemia and years of dialysis(P<0.05).There were no significant differences in hypoglycemic fear scores among patients with different ages,marital status,educational level,gender,blood glucose before dialysis,living style,per capita monthly family income,body mass index(BMI)and diabetes course of DN-MHD(P>0.05).Multivariate analysis showed that frequency of hypoglycemia in the past month,anxiety,severity of hypoglycemia or above,and dialysis years were influencing factors of hypoglycemic fear in DN-MHD patients(P<0.05).Conclusion:The hypoglycemia fear in patients with DN-MHD is higher,which is affected by the frequency of hypoglycemia in the past month,anxiety,severity of hypoglycemia and dialysis years.
