中医药治疗药物性肝损伤的用药规律分析

Analysis of the Medication Rule of Traditional Chinese Medicine for Drug-induced Liver Injury

目的:运用数据挖掘技术分析中医药治疗药物性肝损伤(DILI)的用药规律。方法:以中国知网为数据来源,以“药物性肝损伤”或“药物性肝病”或“药物性肝损害”或“药物性肝炎”或“药源性肝损伤”或“药源性肝病”或“药源性肝损害”为检索词,筛选近二十年中医药治疗DILI的相关文献,将符合标准文献处方录入Excel中,再利用SPSS Modeler 18.0和SPSS Statistics 25.0软件对数据进行关联规则和系统聚类分析。结果:检索共筛选出符合标准文献75篇。纳入治疗DILI的处方75首,涉及123味中药,使用频数782次,其中甘草(51次,68.00%),柴胡(41次,54.67%),白术(36次,48.00%)等使用频率较高。统计分析药物频次>5次的中药,结果显示,治疗DILI的39味中药,药性以寒性为主,药味以苦、甘味为主,药物归经多归于肺、肝、脾经。DILI相关高频中药有甘草、柴胡、白术、白芍、茯苓、当归、茵陈,常用药物组合为柴胡-白芍、柴胡-丹参、甘草-赤芍、甘草-栀子等。结论:中医学从湿热毒瘀角度论治DILI,临床疗效显著,深入挖掘其用药规律有助于护肝新药的研究与开发。

Objective:To use data mining technology to analyze the medication rules of traditional Chinese medicine in the treatment of drug-induced liver injury(DILI).Method:Using CNKI as the data source,“drug-induced liver injury”or“drug-induced liver disease”or“drug-induced liver injury”or“drug-induced hepatitis”or“drug-induced liver injury”or“drug-induced liver disease”“or“drug-induced liver injury”as the search terms,screened the relevant literature on the treatment of DILI with traditional Chinese medicine in the past two decades,entered the prescriptions that conformed to the standard literature into Excel,and then used SPSS Modeler 18.0 and SPSS Statistics 25.0 software to carry out association rules for the data and system cluster analysis.Results:A total of 75 literature meeting the criteria were screened out.75 prescriptions were included in the treatment of DILI,involving 123 traditional Chinese medicines,and the frequency of use was 782 times,among which licorice(51 times,68.00%),Bupleurum(41 times,54.67%),and Atractylodes macrocephala(36 times,48.00%)were used more frequently than others.high.Statistical analysis of traditional Chinese medicines with drug frequency>5 times showed that the 39 traditional Chinese medicines for the treatment of DILI were mainly cold in nature,bitter and sweet in taste,and mostly belonged to the lung,liver and spleen meridian.The highfrequency traditional Chinese medicines related to DILI include licorice,Bupleurum,Atractylodes macrocephala,white peony root,poria cocos,angelica,and capillary.Conclusion:Traditional Chinese medicine treats DILI from the perspective of dampness,heat,poison and blood stasis,and the clinical curative effect is remarkable.Deeply digging its medication rules is helpful for the research and development of new liver-protecting drugs.
