circRNA-CDR1as与AMI患者并发室性心律失常的相关性

摘要 目的:研究环状RNA(circRNA)-小脑变性相关蛋白1反义转录物(CDR1as)与急性心肌梗死(AMI)患者并发室性心律失常的相关性。方法:依据是否并发室性心律失常,于我院治疗的148例AMI患者被分为心律失常组(46例)与无心律失常组(102例)。比较两组一般临床资料及血浆circRNA-CDR1as水平,分析AMI患者并发室性心律失常的影响因素及circRNA-CDR1as与临床指标的相关性。结果:与无心律失常组比较,心律失常组心率、血清B型尿钠肽(BNP)水平、全球急性冠状动脉事件注册(GRACE)评分、主要不良心血管事件发生率、血浆circRNA-CDR1as水平均显著升高,LVEF显著降低(P<0.05或<0.01)。多因素Logistic回归分析显示,circRNA-CDR1as、心率、BNP、GRACE评分均为AMI患者并发室性心律失常的独立危险因素(OR=1.030~20.405,P<0.05或<0.01),LVEF为其独立保护因素(OR=0.930,P=0.002)。Pearson相关性分析显示,circRNA-CDR1as与心率、BNP、GRACE评分呈显著正相关(r=0.780~0.928,P均=0.001),与LVEF呈显著负相关(r=-0.822,P=0.001)。结论:AMI合并室性心律失常患者血浆circRNA-CDR1as水平显著升高,且其为AMI患者并发室性心律失常的独立危险因素。 Objective:To study correlation between circular RNA(circRNA)-cerebellar degeneration-related protein 1 antisense(CDR1as)and ventricular arrhythmia in patients with acute myocardial infarction(AMI).Methods:According to complicated ventricular arrhythmia or not,a total of 148 AMI patients treated in our hospital were divided into arrhythmia group(n=46)and no arrhythmia group(n=102).General clinical data and plasma circRNA-CDR1as level were compared between two groups,and influencing factors of ventricular arrhythmia in AMI patients and correlation between circRNA-CDR1as and clinical indexes were analyzed.Results:Compared with no arrhythmia group,there were significant rise in heart rate(HR),serum level of brain natriuretic peptide(BNP),Global Registry of Acute Coronary Events(GRACE)score,incidence rate of major adverse cardiovascular events and plasma circRNA-CDR1as level,and significant reduction in LVEF in arrhythmia group(P<0.05 or<0.01).Multifactor Logistic regression analysis indicated that circRNA-CDR1as,HR,BNP and GRACE score were independent risk factors for ventricular arrhythmia in AMI patients(OR=1.030~20.405,P<0.05 or<0.01),and LVEF was its independent protective factor(OR=0.930,P=0.002).Pearson correlation analysis indicated that circRNA-CDR1as was significant positively correlated with HR,BNP and GRACE score(r=0.780~0.928,P=0.001 all),and significant inversely correlated with LVEF(r=-0.822,P=0.001).Conclusion:Plasma circRNA-CDR1as level significantly rises in AMI patients complicated ventricular arrhythmia,and it's an independent risk factor for ventricular arrhythmia in AMI patients.
