银屑病患者病耻感现状及其影响因素研究

Study on the status of disease stigma in patients with psoriasis and its influencing factors

[目的]探讨银屑病患者的病耻感现状及其影响因素,以期为识别高危人群、缓解银屑病患者心理负担提供支持。[方法]采用随机抽样的方法,于2021年抽取山东省3所公立医院的银屑病患者167名,运用银屑病疾病耻辱感中文修订版量表开展问卷调查。采用多元线性回归分析探索银屑病患者病耻感的影响因素。[结果]银屑病患者的病耻感总评分为(2.79±0.68)分。病耻感各维度中,积极得分最高为(3.19±1.09)分,敏感与瑕疵得分最低为(2.52±1.03)分。多元线性回归分析显示,瘙痒程度、是否受歧视和乐观心态是病耻感的主要影响因素,其标准化偏回归系数分别为0.164、0.328和-0.157。[结论]银屑病患者的病耻感水平总体不高,瘙痒和受歧视是病耻感的危险因素,乐观心态是病耻感的保护因素。

Objective To investigate the status of disease stigma in patients with psoriasis and its influencing factors and to provide support for identifying high-risk groups and relieving the psychological burden of patients with psoriasis.Methods 167 patients with psoriasis from three public hospitals in Shandong Province were selected in 2021 by random sampling method,and a questionnaire survey was conducted by using the Psoriasis Disease Stigma Chinese Revised Scale.Multiple linear regression analysis was used to explore the influencing factors of disease stigma in patients with psoriasis.Results The total score of disease stigma in patients with psoriasis was(2.79±0.68).Among the dimensions of stigma,the highest score was(3.19±1.09)for the positive dimension and the lowest score was(2.52±1.03)for the sensitive and defective dimension.Multiple linear regression analysis showed that the degree of itching,whether they were discriminated against and whether they were optimistic were the main influencing factors of disease stigma,and their standardized partial regression coefficients were 0.164,0.328 and-0.157 respectively.Conclusions The overall level of stigma in patients with psoriasis is not high.Itching and discrimination are risk factors for disease stigma and optimism is a protective factor for disease stigma.
