针灸结合康复治疗对脑瘫患儿语言障碍的临床观察

Clinical Observation of Acupuncture Combined with Rehabilitation on Language Disorder in Children with Cerebral Palsy

目的 针灸结合康复治疗对脑瘫患儿语言障碍的临床观察。方法 将该院接诊的80例脑瘫患儿随机分为对照组与观察组,各40例。对照组予单纯常规康复治疗,观察组在对照组的基础上联合针灸治疗。两组均在治疗前后根据盖塞尔(Gesell)语言发育商(DQ)和适应性进行评分,且记录对比脑血流灌注、CT变化情况及不良反应。结果 与治疗前比较,对照组和观察组患儿治疗3个月后Gesell语言DQ和适应性评分,均显著改善(P<0.01),且观察组改善程度优于对照组,差异有统计学意义(P<0.01),观察组患儿总有效率90%(36/40),对照组患儿总有效率72.5%(29/40),差异有统计学意义(P<0.05),观察组行治疗后大脑中动脉血流速度值(VP)和血流阻力指数(RI),较治疗前均显著降低(P<0.01),疗效优于对照组(P<0.01)。结论 应用针灸结合康复治疗脑瘫患儿语言障碍,不仅能提高临床疗效,还能有效的改善患儿语言能力。

Objective To observe the clinical effect of acupuncture and rehabilitation therapy on language disorder in children with cerebral palsy.Methods Eighty children with cerebral palsy were randomly divided into control group and observation group, with 40 cases in each group.The control group was treated with simple routine rehabilitation treatment, and the observation group was treated with acupuncture and moxibustion on the basis of the control group.Both groups were scored according to Gesell language development quotient(DQ)and adaptability before and after treatment, and the changes of cerebral blood flow perfusion, CT and adverse reactions were recorded and compared.Results Compared with those before treatment, Gesell language DQ and adaptability scores of children in the control group and the observation group were significantly improved after 3 months of treatment(P<0.01),and the improvement degree of the observation group was better than that of the control group(P<0.01).The total effective rate of children in the observation group was 90%(36/40),and that in the control group was 72.5%(29/40),and the difference was statistically significant(P<0.05).After treatment, the middle cerebral artery blood flow velocity(VP)and blood flow resistance index(RI)in the observation group were significantly lower than those before treatment(P<0.01),and the curative effect was better than that in the control group(P<0.01).Conclusion The application of acupuncture combined with rehabilitation in the treatment of language disorders in children with cerebral palsy can not only improve the clinical efficacy, but also effectively improve the language ability of children.
