不同乳腺癌组织SWV值及对腋窝淋巴结转移的定量预警 被引量:3

摘要 目的探讨不同病理特征的乳腺癌组织剪切波速度(Shear Wave Velocity,SWV)值及对腋窝淋巴结转移的定量预警价值。方法选取2019年1月至2021年10月的105例乳腺癌患者为研究对象。对比不同病理特征患者SWV值及离散系数,建立腋窝淋巴结转移的Logistic回归方程,并进行Hosmer-Lemeshow拟合优度检验,采用受试者工作特征(Receiver Operating Characteristic,ROC)曲线分析SWV值评估腋窝淋巴结转移的效能。结果SWVmean、SWVmax、离散系数与肿瘤直径、分型、组织学分级、腋窝淋巴结转移、人表皮生长因子受体-2及增殖抗原表达有关(P<0.05);肿瘤直径、组织学分级、SWVmean、SWVmax及离散系数与患者腋窝淋巴结转移率有关(P<0.05)。SWVmean及离散系数是腋窝淋巴结转移率的独立危险因素(P<0.05)。ROC分析结果显示,SWVmean、离散系数联合预测腋窝淋巴结转移的ROC曲线下面积值大于SWVmean(χ^(2)=2.210,P=0.027)、离散系数(χ^(2)=2.685,P=0.007)单一指标预测值。结论SWV与乳腺癌病理特征有关,SWVmean、离散系数是腋窝淋巴结转移的独立危险因素,二者联合可提升乳腺癌患者腋窝淋巴结转移评估效能。 Objective To investigate the tissue shear wave velocity(SWV)value of breast cancer with different pathological characteristics and its early warning value for axillary lymph node metastasis.Methods A total of 105 BC patients from January 2019 to October 2021 were selected as the research objects.The SWV value and dispersion coefficient of patients with different pathological characteristics were compared.The Logistic regression equation of axillary lymph node metastasis was established and the Hosmer-Lemeshow goodness of fit test was performed.Receiver operating characteristic(ROC)curve was used to analyze the effectiveness of SWV in evaluating axillary lymph node metastasis.Results The SWVmean,SWVmax,dispersion coefficient were related to tumor diameter,type,histological grade,axillary lymph node metastasis,human epidermal growth factor receptor-2 and proliferation antigen expression(P<0.05).Tumor diameter,histological grade,SWVmean,SWVmax and dispersion coefficient were related to the rate of axillary lymph node metastasis(P<0.05).SWVmean and dispersion coefficient were independent risk factors for axillary lymph node metastasis rate(P<0.05).ROC analysis results showed that the area under curve value of SWVmean and the dispersion coefficient combined to predict axillary lymph node metastasis was greater than the single index prediction value of SWVmean(χ2=2.210,P=0.027)and the dispersion coefficient(χ2=2.685,P=0.007).Conclusion SWV is associated with breast cancer pathological characteristic.SWVmean and dispersion coefficient are independent risk factors of axillary lymph node metastasis.The combination of the two can improve the evaluation performance of axillary lymph node metastasis in breast cancer patients.
