地屈孕酮片联合宫腔镜手术治疗子宫内膜息肉患者的价值及复发率分析

Value and recurrence rate of dydrogesterone tablets combined with hysteroscopic surgery in patients with endometrial polyps

目的探讨地屈孕酮片联合宫腔镜手术治疗子宫内膜息肉患者的价值及复发率。方法选取92例子宫内膜息肉患者,根据随机数字表法分为对照组与试验组,每组46例。对照组单行宫腔镜手术,在此基础上,试验组服用地屈孕酮片治疗。对比两组的临床疗效、子宫内膜、月经情况、性激素、其他血清指标、并发症发生情况及复发率。结果试验组治疗总有效率为95.65%,明显高于对照组的73.91%(P<0.05)。试验组治疗后子宫内膜厚度、月经量均低于对照组,血红蛋白(Hb)水平明显高于对照组(P<0.05)。试验组治疗后卵泡刺激素(FSH)、雌二醇(E_(2))、血管内皮生长因子(VEGF)水平均低于对照组,孕酮(P)水平高于对照组(P<0.05)。试验组宫腔粘连、阴道出血、乳房胀痛、腹痛等并发症发生率以及复发率均低于对照组(P<0.05)。结论地屈孕酮片联合宫腔镜手术治疗子宫内膜息肉的效果显著,能有效抑制子宫内膜增厚,调整性激素水平,减少月经量,降低并发症发生率及复发率,值得临床推广与应用。

Objective To investigate the value and recurrence rate of dydrogesterone tablets combined with hysteroscopic surgery in the treatment of endometrial polyps.Methods 92 patients with endometrial polyps were selected and divided into control group and experimental group according to random number table method,with 46 cases in each group.The control group received hysteroscopic surgery,and the experimental group received dydrogesterone tablets.The clinical efficacy,endometrium,menstruation,sex hormones,other serum indexes,complications and recurrence rate of the two groups were compared.Results The total effective rate of experimental group was 95.65%,which was significantly higher than that of control group(73.91%)(P<0.05).After treatment,endometrial thickness and menstrual volume of experimental group were lower than that of control group,and hemoglobin(Hb)level was significantly higher than that of control group(P<0.05).After treatment,the levels of follicle stimulating hormone(FSH),estradiol(E_(2))and vascular endothelial growth factor(VEGF)in experimental groups were lower than those in control group,while the level of progesterone(P)was higher than that in control group(P<0.05).The rates of complications such as intrauterine adhesion,vaginal bleeding,breast tenderness,abdominal pain and recurrence in experimental group were lower than those in control group(P<0.05).Conclusion Dydrogesterone tablets combined with hysteroscopic surgery have significant effects in the treatment of endometrial polyps,which can effectively inhibit endometrial thickening,adjust sex hormone levels,reduce menstrual volume,reduce the incidence of complications and recurrence rate,and is worthy of clinical promotion and application.
