某市三级公立医院抑郁症患者住院费用关联度分析

Correlation Analysis of Hospitalization Expenses of Patients with Depression in a Tertiary Public Hospital in a City

目的 分析影响某市三级公立医院抑郁症患者住院费用的主要因素,探讨控制费用的途径。方法 收集某院2017年1月1日-2019年12月31日第一诊断符合ICD-10中抑郁障碍的诊断标准,共纳入患者8450例,运用灰色关联分析法分析各费用项目对总费用的影响程度。结果 3年来抑郁症患者的住院费用项目在总体顺位上保持相对稳定。检查费对住院费用的关联度最大(1.0000)、治疗费第2(0.9603)、检验费第3(0.8914),其余依次是床位费(0.8837)、护理费(0.8595)、药费(0.8546)、诊查费(0.8474)、材料费(0.8126)。结论 检查费是影响抑郁症患者住院费用的主要因素,检验费、治疗费是不可忽视的影响因素。加强检查费、治疗费、检验费的管控,协同医疗服务价格调整、医保支付方式改革,可有效减少抑郁症患者住院费用。

Objectives This study aims to analyze the main factors affecting the hospitalization expenses of patients with depression in a tertiary public hospital in a city, and exploring ways to control expenses. Methods A total of 8,450 patients with the first diagnosis met the diagnostic criteria for depressive disorder in ICD-10 from January 1, 2017 to December 31, 2019 were collected from a hospital. Gray correlation analysis was used to analyze the degree of influence of each expense item on the total expense. Results The hospitalization charges of depression inpatients remained relatively stable in last 3 years. The correlation degree of inspection fee to hospitalization fee was the largest(1.0000), treatment fee was in the second(0.9603), inspection fee was in the third(0.8914),and the rest were bed fee(0.8837), nursing fee(0.8595), drug fee(0.8546), diagnosis fee(0.8474), and material fee(0.8126). Conclusions Examination fee is the main factor affecting the hospitalization charges of patients with depression. Inspection fees and treatment fees are factors that cannot be ignored. Strengthening the control of inspection fees, treatment fees, and inspection fees, in conjunction with medical service price adjustments and medical insurance payment reforms, can effectively reduce hospitalization expenses for patients with depression.
