某三甲综合医院2017年-2021年糖尿病患者住院费用结构变化及影响因素分析

Analysis of Structural Changes and Influencing Factors of Hospitalization Expenses of Diabetic Patients in a Tertiary General Hospital from 2017 to 2021

目的 分析某三甲综合医院糖尿病住院患者的基本情况和住院费用结构、变化趋势,为控制医疗费用不合理增长和提高医疗质量提供决策支持。方法 将2017年1月1日-2021年12月31日某院住院病案首页主要诊断为E10-E14的所有病案共计6134份。分别对年龄、性别、是否具有并发症、疾病类型、医保类别、住院天数、药品费用等关键指标进行卡方检验、单因素和多元线性回归分析,比较不同指标对糖尿病患者住院费用的影响。结果 6134例出院患者平均住院费用8007.52元、平均住院日8.6天。女性患者3156例(51.45%),男性患者2978例(48.55%),女性患者略多于男性患者,并且高发于40岁以上的人群。2017年-2021年出院患者数分别为1242例、1249例、1313例、1155例、1175例。多元线性回归分析结果显示年龄、住院天数、是否具有并发症是影响患者住院费用的主要因素,共同解释住院费用变异的80.4%。结论 糖尿病患者住院费用与年龄、是否具有并发症、住院天数密切相关。

Objectives This study aims to analyze the basic situation and the structure and trend of hospitalization expenses of diabetes inpatients in a tertiary general hospital, in order to provide decision support for controlling unreasonable growth of medical expenses and improving medical quality. Methods From January 1, 2017to December 31, 2021, a total of 6134 medical records with the main diagnosis of E10-E14 on the front pages of discharge medical records from a hospital were collected. Chi-square test, univariate and multiple linear regression analysis were performed on key indicators such as age, gender, complication, disease type, medical insurance category, length of hospital stays, and drug costs, respectively, to compare the effects of different indicators on the hospitalization costs of diabetic patients. Results The average hospitalization expenses of 6134 discharged patients were 8007.52 yuan, and the average hospitalization days were 8.6 days. There were 3156 female patients(51.45%)and 2978 male patients(48.55%).There were slightly more female patients than male patients, and the incidence was higher in people over 40 years old. The numbers of discharged patients from 2017 to 2021 were 1242, 1249,1313, 1155, and 1175, respectively. The results of multiple linear regression analysis showed that age, length of hospital stay and whether there were complications were the main factors affecting the hospitalization cost of patients, which together explained 80.4% of the variance of hospitalization cost. Conclusions The hospitalization cost of diabetic patients is closely related to age, complications and length of hospital stay.
