梅毒患者病耻感与心理状况、生活质量的相关性

Correlation between stigma and psychological status and quality of life in patients with syphilis

目的探讨梅毒患者病耻感与心理状况、生活质量的相关性。方法选取2019年5月至2021年5月期间于潍坊市益都中心医院接受治疗的梅毒患者84例为观察组;并于同时期选取80例健康志愿者作为对照组,评估对比两组患者相关指标,并采用Pearson分析各评分间的相关性。结果观察组患者社会影响量表(SIS)总分为(67.12±10.99)分,观察组患者的生活质量综合评定问卷(GQOL-74)各项因子评分均低于对照组(均P<0.05),焦虑自评量表评分(SAS)、抑郁自评量表评分(SDS)均高于对照组(均P<0.05);梅毒患者SIS量表因子社会隔离、社会排斥、内在羞耻以及总分均与GQOL-74各项因子均呈负相关(均P<0.05),与SAS和SDS呈正相关(均P<0.05);多元线性回归分析显示,对于梅毒的病耻感,社会社交情况、心理状态、躯体功能、生活质量具有显著负向影响(P<0.05),SAS、SDS为显著正向影响(均P<0.05)。结论梅毒患者普遍存在病耻感,且其病耻感会导致患者生活质量的下降,与患者的抑郁、焦虑程度也存在一定相关性,临床中可通过改善患者的病耻感以改善其生活质量和心理状态,提高患者的治疗效果。

Objective To explore the correlation between stigma and psychological status and quality of life in patients with syphilis.Methods A total of 84 syphilis patients treated in Yidu Central Hospital of Weifang from May 2019 to May 2021 were selected as the observation group.In the same period,80 healthy volunteers were selected as the control group.The relevant indicators of the two groups were evaluated and compared,and Pearson was used to analyze the correlation between each score.Results The total score of the Social Impact Scale(SIS)in the observation group was(67.12±10.99),and the scores of Generic Quality Of Life Inventory(CQOL-74)were lower than those in the control group(P<0.05).The Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)of the observation group were higher than those of the control group(P<0.05).The scores of social isolation,social exclusion and intrinsic stigma,and total score of SIS were negatively correlated with GQOL-74 factors(P<0.05),while positively correlated with SAS and SDS(P<0.05).Multiple linear regression analysis showed that social status,mental state,physical function and quality of life had significant negative effects on stigma of syphilis(P<0.05),while SAS and SDS had significant positive effects(P<0.05).Conclusions Stigma is common in patients with syphilis,and its stigma can lead to the decline of patients'quality of life,and there is a certain correlation with the degree of depression and anxiety of patients.Clinical improvement of patients'stigma can improve their quality of life and psychological state,and improve the therapeutic effect of patients.
