Glazer评估未育女性盆底功能障碍性疾病的康复治疗效果及临床意义

Glazer's evaluation of rehabilitation treatment for pelvic floor dysfunction in nulliparous women

目的探讨未育女性盆底功能障碍性疾病(PFD)康复治疗的效果及临床意义。方法选择孕前行盆底筛查的未育女性532例,筛查出存在PFD的女性168例作为研究对象,将其分为Kegel治疗组90例和盆底功能性磁刺激治疗组78例,应用Glazer盆底表面肌电评估系统检测其治疗前后的盆底肌功能,分析未育女性PFD的高危因素及治疗前后的效果。结果年龄、体重指数(BMI)、有无慢性咳嗽病史的女性中,PFD的发生率差异有显著性(P<0.05)。Kegel治疗组和盆底功能性磁刺激治疗组与治疗前比较,治疗后快速收缩阶段最大峰值、持续收缩阶段平均值、耐力收缩阶段平均值均升高,且盆底功能性磁刺激治疗组较Kegel组高,差异均有统计学意义(P<0.05)。结论年龄、肥胖、慢性咳嗽史是未育女性发生PFD的高危因素,孕前女性的盆底肌功能可能影响产后PFD的发生率,孕前及早干预是必要的。

Objective To discuss the effect and clinical significance of rehabilitation treatment for pelvic floor dysfunction(PFD)in nulliparous women.Methods A total of 532 nulliparous women who participated in pre-pregnancy examination,and 168 women with PFD were screened as the research objects.They were divided into Kegel treatment group 90 cases and pelvic floor functional magnetic stimulation treatment group 78 cases.The Glazer pelvic floor surface electromyography assessment system was used to detect the pelvic floor muscle function before and after treatment,and to analyze the high-risk factors of PFD in nulliparous women and the effect before and after treatment.Results There were significant differences in the incidence of PFD among women with age,body mass index(BMI)and history of chronic cough(P<0.05).Compared with before treatment,the maximum value of rapid contraction,the average value of continuous contraction and the average value of tolerance test increased in Kegel treatment group and pelvic floor functional magnetic stimulation treatment group,and the difference was statistically significant(P<0.05).Conclusion The age,obesity and history of chronic cough are the high risk factors of PFD in nulliparous women.The pelvic floor muscle function of women before pregnancy may affect the incidence of postpartum PFD.Early intervention before pregnancy is necessary.
