第三代双源双能CT血管造影对自发性蛛网膜下腔出血诊断价值分析

摘要 目的探究第三代双源双能CT血管造影(DSDECTA)对自发性蛛网膜下腔出血(SSAH)诊断价值。方法选取SAH患者58例,所有患者在外科或血管内治疗之前均行DSDECTA及数字减影血管造影(DSA),通过分析DSDECTA和DSA患者阳性检出率、动脉瘤测量数据的差异性来探究DSDECTA对SSAH患者的诊断价值。结果DSDECTA与DSA对SSAH病因检出率对比,差异无统计学意义(P>0.05);对颅内动脉瘤检出率对比,差异无统计学意义(P>0.05);2组检查在不同部位动脉瘤检出及动脉瘤测量数值方面有很好的一致性(P>0.05)。结论第三代DSDECTA对SSAH诊断效果与DSA相近,具有准确、快速、无创和低剂量的特点,可以清晰显示颅内动脉瘤形态、大小及周围组织解剖等情况,能够为外科夹闭术或血管内修复术提供重要决策信息,可以作为SSAH诊断的主要工具。 Objective To explore the value of the third generation dual source dual energy CT angiography(DSDECTA)in the diagnosis of spontaneous subarachnoid hemorrhage(SSAH).Methods Fifty-eighy patients with SSAH were selected.All patients underwent DSDECTA and digital subtraction angiography(DSA)before surgery or endovascular treatment.The diagnostic value of DSDECTA in patients with SSAH was explored by analyzing the difference in positive detection rate and aneurysm measurement data between DSDECTA and DSA.Results Compared with DSA,DSDECTA had a similar effect in detecting the etiology of SSAH(P>0.05).There was no difference in the detection rate of intracranial aneurysms(P>0.05).There was good agreement between the two groups in the detection of aneurysms in different parts and the measured values of aneurysms(P>0.05).Conclusion The diagnostic effect of the third generation DSDECTA on SSAH is similar to that of DSA.It is accurate,rapid,noninvasive and low-dose.It can clearly display the shape,size and surrounding tissue anatomy of intracranial aneurysms.It can provide important decision information for surgical clipping or endovascular repair and can be used as the main tool for the diagnosis of SSAH.
