脑卒中患者经济毒性现状及影响因素分析

The current situation of financial toxicity among stroke patients and its influencing factors

目的 探讨脑卒中患者经济毒性现状,并分析其影响因素。方法 选取2020年6月-2021年6月笔者所在医院收治的287例脑卒中患者作为研究对象,采用一般情况调查表、经济毒性综合评分量表(comprehensive scores for financial toxicity based on the patient-reported outcome measures,COST-PROM)、Barthel指数(Barthel index,BI)、社会支持评定量表(social support rating scale,SSRS)对其进行调查。结果 脑卒中患者经济毒性得分为(22.16±3.83)分,其中得分<26分的患者233例(81.18%)。多元逐步回归分析显示:年龄、家庭人均月收入、日常生活活动能力、客观支持是脑卒中患者经济毒性的独立影响因素(R~2=0.598,_(△)R~2=0.592,F=104.838,P<0.001)。结论 脑卒中患者普遍存在着高水平的经济毒性,医护人员应重点关注年龄<60岁、家庭人均月收入低、日常生活活动能力和客观支持较差的患者,鼓励患者主动寻求家庭和社会的支持,积极参与疾病康复锻炼,使其更好地回归家庭和社会,从而降低患者的经济毒性。

Objective To investigate the current situation of financial toxicity in stroke patients and analyze its influencing factors.Methods A total of 287 stroke patients admitted to our hospital between June 2020 and June 2021 were selected and surveyed using a general condition questionnaire,comprehensive scores for financial toxicity based on the patient-reported outcome measures(COST-PROM),Barthel index(BI)and social support rating scale(SSRS).Results The COST-PROM score was 22.16±3.83,and 233 patients(81.18%)had scores no less than 26.Multiple stepwise regression analysis showed that age,family per capita monthly income,activities of daily living,and objective support were the influencing factors of financial toxicity of stroke patients(R 2=0.598,△R 2=0.592,F=104.838,P<0.001).Conclusion Stroke patients generally have a high level of economic toxicity.Medical staff should pay special attention to patients aged lower than 60 years old,with low family per capita monthly income and poor activities of daily living,and encourage patients to seek family and social support,participate in disease rehabilitation exercises,so as to return to normal family and social life earlier.
