基于加速康复外科肺结节患者胸腔镜术后出院准备度现状与影响因素分析

Study on the status quo and influencing factors of discharge readiness of pulmonary nodules patients after thoracoscopic surgery based on enhanced recovery after surgery

目的:了解肺结节患者胸腔镜术后的出院准备度现状及影响因素,为改善肺结节患者胸腔镜术后的出院准备度提供依据。方法:采用一般资料调查表、出院准备度量表和出院指导质量量表对2022年3月至6月广州医科大学附属第一医院的171例胸腔镜术后肺结节患者进行调查。结果:研究纳入的171例肺结节患者胸腔镜术后的出院准备度总分为96.00(81.00,105.00)分,其中适应维度的标准化均分最高,为9.50(8.00,10.00)分,个人状态维度的标准化均分最低,为6.33(5.00,7.67)分。多元线性回归分析示,疼痛程度、住院天数、出院指导质量是影响胸腔镜术后患者出院准备度的主要因素(均P<0.05),解释总变异的42.6%。结论:肺结节患者胸腔镜术后出院准备度处于中等水平,护理人员应进一步完善出院指导,加强医患互动,提高患者疾病自我管理能力,帮助其树立恢复健康的信心。

Objective:To explore the status and influencing factors of discharge readiness of pulmonary nodule patients after thoracoscopic surgery and improve discharge readiness of pulmonary nodule patients undergoing thoracoscopic surgery.Method:A total of 171 pulmonary nodule patients after thoracoscopic surgery were selected as the survey subjects,and the general information questionnaire,readiness for hospital discharge scale,and discharge guidance quality scale were used to evaluate the patient’s readiness for discharge and the quality of discharge guidance.Result:The total score of hospital discharge readiness for pulmonary nodule patients after thoracoscopic surgery was 96.00(81.00,105.00).The expected support status got the highest score of 9.50(8.00,10.00)and the personal status got the lowest score of 6.33(5.00,7.67).Multiple linear regression analysis showed that pain level,length of stay,and quality of discharge guidance were the independent influencing factor of the discharge readiness of pulmonary nodules patients after thoracoscopic surgery(P<0.05),which could explain 42.6%of the total variation.Conclusion:The discharge readiness of pulmonary nodule patients undergoing thoracoscopic surgery is at a medium level.Nursing staff should further improve discharge guidance,strengthen doctor-patient interaction,improve patients’disease self-management ability and help patients establish confidence in recovering health.
