精准强脉冲光联合长脉冲Nd:YAG激光对玫瑰痤疮患者临床疗效的影响研究

Study on the effect of precise intense pulsed light combined with long pulsed Nd: YAG laser on the clinical efficacy of patients with rosacea

目的 DPL联合长脉冲Nd:YAG激光治疗对玫瑰痤疮患者临床疗效的影响。方法 选取深圳市慢性病防治中心皮肤性病科于2020年1月份到2022年12月份收治的玫瑰痤疮患者110例为研究主体,按照治疗方法的不同,将其分成对照组(给予DPL治疗)和研究组(在对照组基础上联合长脉冲Nd:YAG激光治疗),各55例。观察比较患者的斑评估量表(CEA)、研究者整体评价(IGA)、患者自身评价(PSA)评分以及临床疗效和不良反应。结果 随着治疗次数增加,全部患者的CEA评分和均IGA评分呈现降低趋势,两种亚型患者不同疗法后的CEA评分差异不显著;ETR型患者在第3次治疗后,研究组IGA评分得分低于对照组,PPR型患者每次治疗后,研究组IGA评分得分均低于对照组,且差异显著均显著(P均<0.05);治疗1次时,不同亚型患者给予不同治疗方法后的临床疗效无显著差异;随着治疗次数增加,不同亚型患者的临床总有效率呈现升高趋势,且ETR型患者在第2次、第3次治疗后,研究组临床总有效率高于对照组,且差异显著均显著(P均<0.05);研究组治疗2次和3次的总有效率分别为40.00%和60.00%,均高于对照的21.82%和32.73%,且差异显著均显著(P均<0.05);治疗后,ETR型和PPR型患者中的研究组PSA评分均高于对照组,且差异显著均显著(P均<0.05)。结论 采用DPL联合长脉冲1064nmNd:YAG激光疗法治疗玫瑰痤疮能够有效改善患者临床症状,提升临床综合疗效,且与ETR型患者相比,PPR型患者的临床疗效更好,整体安全较好,值得临床推广。

Objective To investigate the effect of DPL combined with long pulse Nd:YAG laser therapy on the clinical efficacy of patients with rosacea.Methods From January 2020 to December 2022,a total of 110 patients with rosacea admitted to the Department of Dermatology and Venereology of Shenzhen Chronic Disease Prevention and Treatment Center were selected as the study subjects.According to different treatment methods,they were divided into a control group(treated with DPL)and a study group(treated with long pulse Nd:YAG laser on the basis of the control group),each with 55 patients.The plaque assessment scale(CEA),investigator overall assessment(IGA),patient self assessment(PSA)scores,and clinical efficacy and adverse reactions of patients were observed and compared.Results As the number of treatments increased,the CEA scores and average IGA scores of all patients showed a downward trend,and the CEA scores of the two subtypes of patients after different treatments were not significantly different;After the third treatment,the IGA scores of ETR type patients in the study group were lower than those in the control group.After each treatment,the IGA scores of PPR type patients in the study group were lower than those in the control group,with significant differences(P<0.05);There was no significant difference in the clinical efficacy of different subtypes of patients given different treatment methods at the time of treatment;As the number of treatments increased,the total clinical effective rates of patients with different subtypes showed an upward trend,and after the second and third treatment,the total clinical effective rates of patients with ETR type in the study group were higher than those in the control group,with significant differences(both P<0.05);The total effective rates of treatment 2 and 3 in the study group were 40.00%and 60.00%,respectively,higher than those of the control group(21.82%and 32.73%),with significant differences(P<0.05);After treatment,the PSA scores of both ETR and PPR patients in the study group were significantly higher than those in the control group(both P<0.05).Conclusion DPL combined with long pulse 1064nm Nd:YAG laser therapy for rosacea can effectively improve the clinical symptoms of patients with rosacea and enhance clinical comprehensive efficacy.Compared with ETR patients,PPR patients have better clinical efficacy and overall safety,which is worthy of clinical promotion.
