吞咽口肌生物反馈训练结合吞咽低频电刺激对脑卒中后吞咽困难患者的治疗效果观察 被引量:2

摘要 目的:探讨吞咽口肌生物反馈训练结合吞咽低频电刺激对脑卒中后吞咽困难患者的治疗效果。方法:选取2020年10月1日—2021年10月1日暨南大学附属广州红十字医院昌岗康复二区收治的107例脑卒中后吞咽困难患者作为研究对象,对患者进行吞咽口肌生物反馈训练结合吞咽低频电刺激。比较治疗前后患者洼田饮水试验结果、吞咽功能评分、生活质量评分。结果:治疗后,107例患者治疗后洼田饮水试验结果Ⅲ级占比低于治疗前,Ⅰ级+Ⅱ级占比高于治疗前,差异有统计学意义(P<0.05)。107例患者中,痊愈及显效68例,占比63.55%;有效95例,占比88.79%;无效12例,占比11.21%。治疗后,患者吞咽功能评分低于治疗前,差异有统计学意义(P=0.000 1)。治疗后,患者生理功能、生理职能、生命活力、社会功能、角色功能、认知功能、心理健康、总生活质量评分均高于治疗前,差异有统计学意义(P=0.001)。结论:吞咽口肌生物反馈训练结合吞咽低频电刺激治疗脑卒中后吞咽困难的效果显著,能够明显改善患者的吞咽功能。 Objective:To explore the treatment effect of swallowing oral muscle biofeedback training combined with swallowing low-frequency electrical stimulation in the patients with dysphagia after stroke.Methods:From October 01,2020 to October 01,2021,107 patients with dysphagia after stroke in Changgang Rehabilitation Zone 2,Guangzhou Red Cross Hospital Affiliated to Jinan University were selected as the study subjects.The patients were given swallowing oral muscle biofeedback training combined with swallowing low-frequency electrical stimulation.The results of the water swallow test,swallowing function score,and quality of life score before and after treatment were compared.Results:After treatment in 107 patients,the proportion of grade III in the water swallow test after treatment was lower than that before treatment,and the proportion of grade I+II was higher than that before treatment,and the difference was statistically significant(P<0.05).Among the 107 patients,68 cases were cured and had significant response,accounting for 63.55%;95 cases had a response,accounting for 88.79%;12 cases had no response,accounting for 11.21%.After treatment,the swallowing function score was lower than that before treatment,with statistically significant difference(P=0.0001).After treatment,the scores of physiological function,physiological functioning,life vitality,social function,role function,cognitive function,mental health and total score of quality of life were higher than those before treatment,and the difference was statistically significant(P=0.001).Conclusion:Swallowing oral muscle biofeedback training combined with swallowing low-frequency electrical stimulation is effective in the treatment of dysphagia after stroke,which can significantly improve swallowing function of patients.
