HIV单阳家庭生命质量现况及影响因素调查

Status quo and Influencing factors of quality of life in HIV sero-discordant couples

目的调查HIV单阳家庭夫妻双方生命质量现况及其影响因素。方法采用自制问卷和健康调查简易问卷(SF-36),对夫妻双方进行生命质量调查,分析影响因素,并与常模进行比较。结果阳性方生理健康(PSS)得分(68.61±16.36)分,心理健康(MSS)得分(64.52±17.29)分;阴性方PSS(79.11±15.60)分,MSS(78.24±15.52)分,其中在躯体疼痛(BP)、一般健康状况(GH)、精力(VT)、社会功能(SF)、精神健康(MH)、PSS及MSS,HIV单阳家庭阳性方得分低于阴性方及常模,差异有统计学意义(P<0.05)。在GH、VT、SF、MH及MSS领域,阴性方得分高于常模,差异有统计学意义(P<0.05)。单阳家庭中阳性方未出现HIV感染临床及合并症状,感染后夫妻关系好是PSS得分的保护因素;感染后夫妻关系好是MSS得分的保护因素。单阳家庭阴性方年轻和高收入是PSS得分的保护因素;高收入和感染后夫妻关系好是MSS得分的保护因素。结论应关注单阳家庭阳性方的生理、心理健康问题,加大对单阳家庭阳性方的关怀和支持。建议定期做好单阳家庭的随访与健康管理工作,加强与单阳家庭的交流与联系,鼓励夫妻双方加强沟通,改善夫妻关系,以全面提升单阳家庭生命质量。

Objective To investigate the status and influencing factors of quality of life in HIV sero-discordant couples.Methods A self-designed questionnaire and Health Survey(SF-36)short form were used to investigate the quality of life of the HIV sero-discordant couples.Influencing factors were analyzed and compared with the norm.Results The physical status score of the positive couple is 68.61±16.36,with the mental status score 64.52±17.29.The physical status score of the negative couple is 79.11±15.60,with the mental status score of 78.24±15.52.The average scores of body pain,general health,vitality,social functioning,mental health,physical status score and mental status score among positive couple were lower than the negative side and norm,the difference was statistically significant(P<0.05).The scores of general health、vitality、social functioning、mental health and mental status score among negative couples were higher than the norm(P<0.05).Positive couple without clinical symptoms and having good relationship with negative couple are protective factors for physical health scores.Good marital relationship between couples were protective factors for mental health scores.Negative couples with young and high income were the protective factors for health scores.Negative couples with high income and good marital relationships were protective factors for mental health scores.Conclusions The scores of quality of life in multiple domains were decreased in HIV sero-discordant couples.Therefore,we should pay attention to increase care and support,strengthen communication and help to improve the relationship between the couples in order to be improve the quality of life of the HIV sero-discordant couples.
