绵阳市男男性行为者性施虐/受虐现状及影响因素分析

Current situation and influencing factors of sexual sadism/masochism among men who have sex with men in Mianyang City

目的了解四川省绵阳市MSM性施虐/受虐现状,分析影响因素。方法用滚雪球抽样法招募调查对象,对MSM进行性施虐/受虐和性行为等问卷调查,采用χ^(2)检验、Logistics回归模型分析性施虐/受虐的影响因素。结果回收有效问卷1087份,报告曾有过性施虐/受虐3.6%(39人)。近1年有过性施虐/受虐1.7%(19人),其中31.6%为施虐、26.3%为受虐、42.1%为施虐和受虐均有,26.3%发生过出血,5.3%每次、68.4%有时在性施虐/受虐前(或中)使用过毒品。近1年性施虐/受虐中15.8%每次、68.4%有时发生过肛交,57.9%每次、36.8%有时、5.3%从未使用安全套。多因素分析结果,工人/农民/餐饮服务(OR=2.317)、外县/市/区户籍(OR=4.011)、县级城市居住(OR=3.597)、自我隐瞒中等/高水平(OR=2.397)、近6个月多性伴(OR=2.239)者发生性施虐/受虐可能更大,本地居住时间>2年(OR=0.414)者发生性施虐/受虐的可能更小。结论绵阳市MSM中的性施虐/受虐行为发生率较低,受人口学特征、自我隐瞒水平和性行为等影响,性施虐/受虐者存在多种风险行为叠加,增大了HIV感染风险,要针对性开展行为干预。

Objective To understand the current situation and influencing factors of sexual sadism/masochism among men who have sex with men(MSM)in Mianyang City,Sichuan Province.Methods The snowball sampling was used to recruit subjects and information of sexual sadism/masochism and sexual behavior of MSM were collected by questionnaire.χ^(2)test and logistic regression models were used to analyze the influence factors of sexual sadism/masochism.Results 1087 valid questionnaires were received and 3.6%(39 people)reported previous sexual sadism/masochism,.In the past year,1.7%(19 people)had history of sexual sadism/masochism,31.6%of which are sadistic,26.3%of which are masochistic,42.1%of which are sadistic and masochistic,26.3%have had to bleed.5.3%have been used auxiliaries each time and 68.4%sometimes before(or during)sexual sadism/masochism.In the past year,15.8%had anal sex each time and 68.4%sometimes in sexual sadism/masochism,57.9%had used condom every time,36.8%sometimes,and 5.3%never used condom.Multivariate analysis showed that occupation of workers/farmers/cater services(OR=2.317),non-local county/city/district household registration(OR=4.011),county-level city residence(OR=3.597),self-concealment of medium/high level(OR=2.397),multiple sexual partners(OR=2.239)in the past six months were more likely to have sexual sadism/masochism.Those with a local residence time of>2 years(OR=0.414)were less likely to have sexual sadism/masochism.Conclusions The incidence of sexual sadism/masochism in MSM in Mianyang City was at a low lerel.However,influenced by demographic characteristics,self-conealment lerel,and sexual beharior,sexual sadism/masochism had superimposed multple risk behariors,which increases the risk of HIV infection,suggesting that targeted behavioral interventions should be carried out.
